摘  要:目的探讨鼻咽癌(nasopharyngeal carcinoma,NPC)患者肿瘤组织中浸润性树突状细胞(invasive dendritic cells,IDC)表面分子表达水平及临床价值。方法收集2013年1月-2018年12月湖北省第三人民医院肿瘤科收治的NPC患者86例为观察组,同期收集的鼻息肉患者86例为对照组。分析比较两组患者病变组织中肿瘤IDC表面分子MHC-Ⅱ和CD54表达水平的差异,以及NPC组织中IDC密度、MHC-Ⅱ和CD45表达水平与临床分期、淋巴结转移、肿瘤细胞分化程度的关联。结果与对照组比较,观察组患者肿瘤IDC密度降低,MHC-Ⅱ和CD45阳性表达率均降低,差异有统计学意义(P<0.05)。不同临床分期的鼻咽癌患者肿瘤IDC密度、MHC-Ⅱ和CD54阳性率间比较,差异有统计学意义(P<0.05)。与无淋巴结转移的患者比较,淋巴结转移的患者肿瘤IDC密度、MHC-Ⅱ和CD54阳性率均降低,差异有统计学意义(P<0.05)。肿瘤细胞分化程度为中低分化的患者和高分化的患者肿瘤IDC密度、MHC-Ⅱ和CD54阳性率差异无统计学意义(P>0.05)。结论NPC患者肿瘤组织中IDC多为功能不成熟的细胞,IDC表面分子表达水平降低与患者临床分期和淋巴结转移有关。ObjectiveTo investigate the expression of surface molecules of infiltrating dendritic cells(IDC)in nasopharyngeal carcinoma(NPC)and and to analyze its clinicopathological significance.MethodsFrom January 2013 to December 2018,86 NPC patients admitted to the Oncology Department of the Third People′s Hospital of Hubei Province were selected as the observation group,and 86 nasal polyps patients collected in the same period were set as the control group.The expression levels of MHC-Ⅱ and CD54 on the surface of tumor IDC in the two groups were analyzed and compared.The correlation was also analyzed and compared between the expression levels of IDC density,MHC-Ⅱ and CD45 in NPC and clinical stage,lymph node metastasis and differentiation of tumor cells.ResultsCompared with the control group,the density of tumor IDC and the positive expression rates of MHC-Ⅱ and CD45 in the observation group were significantly lower(P<0.05).There were significant differences in tumor IDC density,MHC-Ⅱ and CD54 positive rates among nasopharyngeal carcinoma patients with different clinical stages(P<0.05).Compared with patients without lymph node metastasis,the density of tumor IDC,the positive rate of MHC-Ⅱ and CD54 in patients with lymph node metastasis were significantly lower(P<0.05).No significant difference can be identified in IDC density,MHC-Ⅱ and CD54 positive rates between patients with moderately or poorly differentiated and highly differentiated tumors(P>0.05).ConclusionIn patients with NPC,the expression of IDC in tumors were strikingly decreased,and most of them were immature cells,which was closely related to clinical stage and lymph node metastasis.
