摘　　要：目的探讨宫颈神经内分泌肿瘤(NTC)的临床、病理特征,并分析其预后的影响因素。方法回顾性分析我科收治的155例宫颈神经内分泌肿瘤患者的临床病理资料。结果155例NTC患者中,随访期内死亡者共89例。Ⅰ~Ⅳ期患者5年生存率分别为66.0%、50.2%、46.1%、0%。中位总生存时间为55.9个月。COX单因素分析显示:NTC的预后与国际妇产科联盟(FIGO)分期、肿瘤大小(>4 cm)、淋巴结转移、宫颈间质深层浸润有关联,与年龄、病理类型、肿瘤成分、宫旁浸润、淋巴脉管浸润、神经浸润、嗜铬素A(CgA)阳性、治疗方式、化疗方案、化疗次数无明显关联。多因素分析显示:FIGO分期、肿瘤大小(>4 cm)是宫颈神经内分泌肿瘤的5年生存率的独立预后影响因素(P均<0.05)。结论FIGO分期、肿瘤大小是影响患者的长期生存的独立预后影响因素。Objective To investigate the clinicopathological characteristics of the neuroendocrine tumor of cervix(NTC)with stageⅠ-Ⅳ,and to analyse its prognostic factors.Methods This study was a retrospective analysis of 155 cases of neuroendocrine tumor of cervix in the department of Gynecological Oncology Surgery.Results Eighty-nine patients died during the follow-up period.The 5-year survival rates of stageⅠ-Ⅳ patients were 66.0%,50.2%,46.1% and 0%,respectively,and the median OS was 55.9 months.The univariate analysis showed that FIGO stage,tumor size(>4 cm),lymph node metastasis and deep invasion in cervical stromal were significant prognostic factors(P<0.05),while age,pathological type,tumor components,parametrical invasion,lymphovascular space invasion,nerve invasion,CgA positive,treatment,chemotherapy regimen and times of chemotherapy were not significantly associated with prognosis(all P>0.05).Multivariate analysis showed that FIGO stage and tumor size(>4 cm)are independent prognostic factors for 5-year survival of NTC(all P<0.05).Conclusion FIGO stage and tumor size(>4 cm)were independent prognostic factors for 5-year survival of NTC.
