米氮平联合养血清脑颗粒治疗脑卒中抑郁的临床研究

摘　　要：目的观察米氮平联合养血清脑颗粒治疗脑卒中抑郁的临床疗效。方法选取2015年8月-2017年8月廊坊市人民医院收治的104例脑卒中抑郁病人进行研究,采用随机数字表法分为研究组与对照组,每组52例。两组病人均给予常规治疗,对照组在此基础上服用米氮平治疗,研究组在对照组基础上口服养血清脑颗粒治疗,观察比较两组病人治疗前后神经功能缺损程度、抑郁状况及失眠情况。结果研究组总有效率为92.31%,明显高于对照组的75.00%,差异有统计学意义(P<0.05)。治疗1周、2周、4周、8周后,研究组与对照组美国国立卫生研究院卒中量表(NIHSS)评分、汉密尔顿抑郁量表(HAMD)评分、匹兹堡睡眠质量指数量表(PSQI)评分明显低于治疗前(P<0.05),且研究组明显低于对照组(P<0.05);两组治疗后日常生活活动能力量表(ADL)评分明显高于治疗前(P<0.05),且研究组明显高于对照组(P<0.05);研究组与对照组血清神经元特异性烯醇化酶(NSE)水平明显低于治疗前(P<0.05),且研究组明显低于对照组(P<0.05);研究组不良反应总发生率为7.69%,对照组不良反应总发生率为9.62%,两组比较差异无统计学意义(χ^2=0.000,P=1.000)。结论米氮平联合养血清脑颗粒治疗脑卒中抑郁病人能明显改善神经缺损情况,提高病人日常生活活动能力,改善抑郁,提高睡眠质量。Objective To observe the effect of Mirtazapine combined with Yangxue Qingnao Granules in the treatment of patients with poststroke depression.Methods One hundred and four cases of poststroke depression patients in Langfang People's Hospital were selected,and divided into the study group and the control group by the random digital table,with 52 cases in each group.The patients in two groups were treated with routine treatment,and the patients in control group was treated with Mirtazapine on the basis of routine treatment,and that in the study group was treated with Yangxue Qingnao Granules on the basis of the control group.The degree of nerve function defect,depression and insomnia condition were observed and compared between the two groups before and after treatment.Results The total effective rate of the study group was 92.31%,wihch was significantly higher than 75.00% of the control group(P<0.05).After treatment for 1 week,2 weeks,4 weeks,and 8 weeks,the National Institutes of Health Stroke Scale(NIHSS)scores,Hamilton Depression Scale(HAMD)scores,Pittsburgh Sleep Quality Index Scale(PSQI)scores in the study group and the control group were significantly lower than those before treatment(P<0.05),and that in the study group were significantly lower than those in the control group(P<0.05).The scores of ADL in the study group and the control group were significantly higher than those before the treatment(P<0.05),the study group was significantly higher than that in the control group(P<0.05).The level of serum neuron specific enolase(NSE)in the study group and the control group was significantly lower than that before the treatment(P<0.05),the study group was significantly lower than that in the control group(P<0.05);The total incidence rate of adverse reactions in the putients in study group was 7.69%,and that in the control group was 9.62%,there was no statistically significant difference between the two groups(χ^2=0.000,P=1.000).Conclusion Mirtazapine combined with Yangxue Qingnao Granules in the treatment of po
