摘　　要：目的:分析Miccoli切除术治疗甲状腺肿瘤的临床效果及安全性。方法:选择2014年1月-2018年1月笔者所在医院收治的820例甲状腺肿瘤患者,按照不同的手术治疗方式分为A组(双侧甲状腺全切术)、B组(单侧甲状腺腺叶切除术)和C组(Miccoli切除术);对比组间术后甲状旁腺激素水平、血清钙水平、低血钙及甲状旁腺功能减退发生率。结果:C组术后1、3、7 d甲状旁腺激素水平均高于A组、B组,且B组高于A组,差异均有统计学意义(P<0.05);C组术后1、3、7 d血清钙水平均高于A组、B组,且B组高于A组,差异均有统计学意义(P<0.05);C组低血钙及甲状旁腺功能减退发生率均低于A组、B组,差异均有统计学意义(P<0.05)。结论:Miccoli切除术治疗甲状腺肿瘤的临床效果显著,可以改善患者的甲状旁腺激素水平、血清钙水平,能够有效降低患者低血钙及甲状旁腺功能减退发生率,应用安全性极高。Objective: To analyze the clinical effect and safety of Miccoli in the treatment of thyroid tumor. Method: A total of 820 patients with thyroid tumors admitted to the hospital from January 2014 to January 2018 were selected. According to different surgical methods, they were divided into the group A(bilateral total thyroidectomy), the group B(unilateral thyroidectomy) and the group C(Miccoli resection). The index of postoperative parathyroid hormone levels, serum calcium levels, and incidence of hypocalcemia and hypoparathyropathy hypocalcemia and hypoparathyroidism among three groups were compared. Result: The levels of parathyroid hormone in the group C were higher than those in the group A and the group B at the 1 st, 3 rd and 7 th day after operation, and the group B was higher than the group A, the differences were statistically significant(P<0.05). The levels of serum calcium in the group C were higher than those in the group A, the group B at the 1 st, 3 rd and 7 th day after operation, and the group B was higher than the group A, the differences were statistically significant(P<0.05). The incidence of hypocalcemia and hypoparathyroidism were lower in group C than in group A and group B, the differences were statistically significant(P<0.05). Conclusion: The clinical effect of Miccoli resection for thyroid neoplasms is significant. It can improve the level of parathyroid hormone and serum calcium in patients, and can effectively reduce the incidence of hypocalcemia and hypoparathyroidism in patients. The safety of application is extremely high.
