摘  要:目的探讨基质金属蛋白酶(matrix metallo proteinase,MMP)-13、白介素-17(interleukin-17,IL-17)、核转录因子(NF-κB)-p65在初发多发性骨髓瘤(multiple myeloma,MM)患者骨髓中的表达水平及预测意义。方法收集76例初发MM患者的临床资料,同时收集同期20例正常骨髓组织作为对照。采用Western blot和实时荧光定量PCR测定骨髓组织中MMP-13、IL-17、NF-κB-p65蛋白和mRNA的表达量。采用Person相关性分析MMP-13、NF-κB-p65与IL-17表达之间的相关性;采用单因素生存分析Kaplan-Meier绘制生存曲线。结果与正常骨髓组织相比,MM患者骨髓组织中MMP-13、IL-17、NF-κB-p65蛋白和mRNA表达量明显上调(P<0.05);MMP-13和IL-17表达与MM患者ISS分期、肌酐、β2-微球蛋白、血小板、血红蛋白和骨髓浆细胞比例有关(P<0.05);相关性分析显示,MMP-13和NF-κB-p65的表达均与IL-17表达呈正相关(r=0.537,P<0.001;r=0.596,P<0.001);生存分析显示,骨髓组织中MMP-13高表达患者的3年总生存率(20.00%)明显低于MMP-13低表达患者(42.86%)(Log-rank χ^2=5.741,P=0.017);骨髓组织中IL-17高表达患者的3年总生存率(17.14%)明显低于IL-17低表达患者(45.71%)(Log-rank χ^2=7.244,P=0.007)。结论MMP-13、IL-17、NF-κB-p65在MM患者骨髓组织中表达量明显上调,MMP-13和IL-17低表达患者具有更高的3年生存率。Purpose To investigate the expression and predictive significance of matrix metallo proteinase-13,interleukin-17 and NF-κB-p65 in the bone marrow of patients with newly diagnosed primary multiple myeloma(MM).Methods Clinical data of 76 patients with new diagnosed MM were collected,and 20 normal bone marrow tissues were collected as controls at the same time.The expression levels of MMP-13,IL-17,NF-κB-p65 protein and mRNA in bone marrow tissues were determined by Western blot and real-time quantitative PCR.The correlation between MMP-13,NF-κB-p65 and IL-17 expression was analyzed by Person correlation.The survival curve was drawn by single factor survival analysis Kaplan-Meier.Results Compared with normal bone marrow tissues,the protein and mRNA expression levels of MMP-13,IL-17 and NF-κB-p65 were significantly up-regulated in bone marrow tissues of MM patients(P<0.05).The expression levels of MMP-13 and IL-17 were related to ISS stage,creatinine,β2-microglobulin,platelet,hemoglobin and bone marrow plasma cell ratio in MM patients(P<0.05).Person correlation analysis showed that the expression of MMP-13 and NF-κB-p65 were positive correlation with the expression of IL-17(r=0.537,P<0.001,r=0.596,P<0.001).Survival analysis showed that the 3-year overall survival rate of patients with high MMP-13 expression in bone marrow tissues was lower than that of patients with low MMP-13 expression(20.00%vs 42.86%)(Log-rank χ^2=5.741,P=0.017).The 3-year overall survival rate of patients with high IL-17 expression in bone marrow tissues was significantly lower than that of patients with low IL-17 expression(17.14%vs 45.71%)(Log-rank χ^2=7.244,P=0.007).Conclusion The expression levels of MMP-13,IL-17 and NF-κB-p65 are significantly up-regulated in bone marrow tissues of MM patients,and patients with low expression of MMP-13 and IL-17 have a higher 3-year survival rate.
