摘  要:目的探讨基于18F-FDG PET/CT的意识障碍患者脑葡萄糖代谢水平。方法选择2017年1月~2018年10月在我院治疗的意识障碍患者80例为研究对象,根据CRS-R评分分为植物状态组60例与微意识状态组20例,另选择健康体检者30例为对照组,均行18F-FDG PET/CT扫描,采用SPM分析系统自动分析脑组织葡萄糖标准摄取值。结果左侧半球:对照组在额叶、顶叶、颞叶、枕叶、丘脑、基底节、小脑18F-FDG标准摄取值均显著高于植物状态组与微意识状态组,差异有统计学意义(P<0.05)。右侧半脑:对照组额叶、顶叶、颞叶、枕叶、丘脑、基底节、小脑区18F-FDG标准摄取值均显著高于植物状态组,差异有统计学意义(P<0.05);微意识状态组额叶、顶叶、颞叶、枕叶、丘脑、基底节区18F-FDG标准摄取值均显著高于植物状态组,差异有统计学意义(P<0.05);对照组小脑区18F-FDG标准摄取值显著高于微意识状态组,差异有统计学意义(P<0.05)。植物状态组脑干标准摄取值为(3.11±1.02),微意识状态组(3.40±1.15),对照组(3.43±1.12),差异无统计学意义(P>0.05)。结论意识障碍患者脑组织各部分葡萄糖代谢水平均显著下降,微意识状态患者在左侧丘脑、基底节部位以及右侧额叶、顶叶、颞叶、枕叶、丘脑、基底节区糖代谢水平显著升高;脑干糖代谢水平没有显著变化。Objective To investigate the level of brain glucose metabolism in patients with disturbance of consciousness based on 18F-FDG PET/CT. Methods A total of 80 patients with disturbance of consciousness who were treated in hospital from January 2017 to October 2018 were enrolled. Eighty patients were divided into 60 groups of vegetative state group and 20 cases of micro-consciousness group according to CRS-R score. Thirty health examiners were selected as the control group and treated with 18F-FDG PET/CT scan. The SPM analysis system was used to automatically analyze the standard uptake value of brain tissue glucose. Results In the left hemisphere: the standard uptake values of 18F-FDG in the frontal, parietal, temporal, occipital, thalamus, basal ganglia and cerebellum were significantly higher than those in the phyto-state group and the micro-conscious state group. The difference was statistically significant(P<0.05).Right hemisphere: The standard uptake values of 18F-FDG in the frontal, parietal, temporal, occipital, thalamus, basal ganglia and cerebellum were significantly higher than those in the phyto-state group, and the difference was statistically significant(P<0.05). The standard uptake values of 18F-FDG in the frontal, parietal, temporal, occipital, thalamus and basal ganglia of the micro-conscious state group were significantly higher than those in the phyto-state group(P<0.05).The standard 18F-FDG standard uptake values in the control group were significantly higher than those of the microconscious state group, and the difference was statistically significant(P<0.05). The standard intake of brainstem was(3.11±1.02) in the plant state group,(3.40±1.15) in the microconscious state group, and(3.43±1.12) in the control group,and the difference was not statistically significant(P>0.05). Conclusion The levels of glucose metabolism in the brain tissue of patients with disturbance of consciousness are significantly decreased.
