摘 要:目的利用高频超声评估非酒精性脂肪肝(NAFLD)患者和健康对照组胰腺脂肪浸润与亚临床动脉粥样硬化之间的关系。方法门诊患者经超声诊断非酒精性脂肪肝患者100例(A组)和健康对照50例(B组)。采用超声腹部凸阵探头进行非酒精性脂肪肝及胰腺是否脂肪浸润的检查,利用高频线阵探头进行颈动脉超声检查测量颈动脉内膜中层厚度(CIMT),探讨NAFLD患者胰腺脂肪浸润与亚临床动脉粥样硬化的关系。结果2组中胰腺脂肪浸润分别为96%、30.5%。A组患者胰腺脂肪浸润分级0级3例(3%),1级39例(39%),2级53例(53%),3级10例(10%);B组中0级34例(68%),1级14例(28%),2级2例(4%)。NAFLD患者新诊断糖尿病和前驱糖尿病的发生率为7%和22%。2组中CIMT分别为(0.470±0.09)mm、(0.428±0.08)mm,差异有统计学意义(P<0.05)。2组中CIMT的水平随着胰腺脂肪浸润分级的增加而增加,二者呈正相关(r=0.385,P<0.001)。结论胰腺脂肪浸润是NAFLD患者动脉粥样硬化发生的一个促成因素。该研究还证实了NAFLD与胰腺脂肪浸润之间的强关联。超声技术能够客观评价患者NAFLD、胰腺是否有脂肪浸润及其分级、CIMT情况,具有重要的临床意义。Objective To investigate the relationship between pancreatic fat infiltration and subclinical atherosclerosis in patients with nonalcoholic fatty liver disease(NAFLD)and healthy subjects by using high frequency ultrasound.Methods A total of 100 outpatients with NAFLD who were diagnosed by ultrasonography were enrolled into group A,at the same time,the 50 healthy subjects were enrolled into group B.Ultrasound examination was performed for the patients in both groups.The ultrasound abdominal convex array probe was used to check the nonalcoholic fatty liver and pancreas for fat infiltration.The grade of pancreatic fat infiltration was differentiated into 0 to 3 grades.The carotid intima media thickness(CIMT)was measured by high frequency linear array probe to investigate the relationship between pancreatic fat infiltration and subclinical atherosclerosis in patients with NAFLD.At the same time,the general data and laboratory indexes were observed and compared between the two groups.Results The pancreatic fat infiltration rates in group A and group B were 96%and 30.5%,respectively.At the aspect of pancreatic fat infiltration grading,there were 3 cases of grade 0(3%),39 cases of grade 1(39%),53 cases of grade 2(53%),and 10 cases of grade 3(10%)in group A.Ontrast of that there were 34 cases of grade 0(68%),14 cases of grade 1(28%)and 2 cases of grade 2(4%)in group B.The incidences of newly diagnosed diabetes and prediabetes in NAFLD patients were 7 and 22%,respectively.There were significant differences in CIMT levels between A and B groups(P<0.05).The levels of CIMT were increased with the increase of pancreatic fat infiltration grade(P<0.05),and there was a positive correlation between them(r=0.385,P<0.01).Conclusion The pancreatic fat infiltration is a contributing factor in the pathogenesis of atherosclerosis in patients with NAFLD.The research results confirm that there is a close correlation between NAFLD and pancreatic fat infiltration.The ultrasound technology can objectively evaluate the fat infiltration and g
