摘  要:目的观察急性后循环脑梗死患者血清胱抑素C(CysC)水平是否与颅内、颅外大动脉粥样硬化相关。方法回顾分析219例住院3 d内发生的急性后循环非心源性脑梗死患者,使用颗粒增强透射免疫比浊法(PETIA)测定血清CysC水平。采用CTA或DSA行头颈部血管检查,计算椎基底动脉狭窄程度;采用多变量logistic回归分析CysC水平与颅内外大动脉粥样硬化的相关性。结果219例患者(平均年龄70.2岁),有大动脉粥样硬化78例(35.6%)。大动脉粥样硬化的分布部位分别是颅外段椎动脉28例,颅内段椎动脉/基底动脉35例(椎动脉8例,基底动脉25例,椎动脉+基底动脉2例)和串联狭窄15例(包括颅内段和颅外段的椎动脉/基底动脉)。与颅内段椎动脉/基底动脉狭窄的患者比较,颅外段狭窄患者CysC水平更高(P<0.01),多变量logistic回归分析显示高水平CysC与颅外段椎动脉粥样硬化有相关性。结论CysC参与了动脉粥样硬化的形成过程,高水平的CysC是脑动脉狭窄的危险因素,急性后循环非心源性脑梗死患者血清CysC水平与颅外段椎动脉粥样硬化相关。Objective To investigate the orrelation between serum cystatin C(CysC)levels and intracranial and extracranial large artery atherosclerosis in patients with acute posterior circulation cerebral infarction.Methods The clinical data about 219 patients with acute posterior circulation non-cardiogenic cerebral infarction who were hospitalized in our hospital within 3 days were retrospectively analyzed.The particle enhanced transmission immunoturbidimetric method(PETIA)was used to detect serum CysC levels.The degree of vertebral basilar artery stenosis was calculated by CTA or DSA.Multivariate logistic regression was used to analyze the correlation between serum CysC levels and intracranial and extracranial large artery atherosclerosis.Results Of the 219 patients(mean age:70.2 years),78 cases(35.6%)had large artery atherosclerosis.The distribution of atherosclerosis was found in extracranial vertebral arteries(28 cases),and intracranial vertebral arteries/basilar arteries(35 cases)including 8 cases in vertebral arteries,25 cases in basilar arteries,2 cases in vertebral arteries+basilar arteries,moreover,15 patients had paiallel stenosis,including intracranial and extracranial vertebral arteries/basilar arteries.The serum levels of CysC in patients with extracranial vertebral/basilar stenosis were significantly higher than those in patients with intracranial vertebral/basilar stenosis(P<0.01).Multivariate logistic regression analysis showed that high levels of CysC were correlated with extracranial vertebral atherosclerosis.Conclusion Cystatin C is involved in the formation of atherosclerosis,and high levels of cystatin C are a risk factor for cerebral artery stenosis.Moreover the serum levels of CysC are correlated with extracranial vertebral atherosclerosis in patients with acute posterior circulation non-cardiogenic cerebral infarction.
