摘　　要：目的:探讨血清LRG1、LDH、β2-MG在弥漫大B细胞淋巴瘤(DLBCL)患者化疗后复发的临床意义。方法:对80例弥漫大B淋巴瘤患者治疗前血清富亮氨酸α-2糖蛋白-1(Leucine-rich-alpha-2-glycoprotein-1,LRG1)、乳酸脱氢酶(lactate dehydrogenase,LDH)、β2-微球蛋白(Beta 2-microglobulin,β2-MG)的水平进行检测并对患者随访。通过受试者工作曲线(ROC)确定无复发生存(RFS)的最佳截断值,采用分析的最佳截断值比较分析3种血清标记物在预测弥漫大B淋巴瘤患者治疗后复发及预后因素的相关性。结果:LDH、LRG1及β2-MG分别在肿瘤分期高、有结外受侵及骨髓受累的分组中有更高的血清水平,差异有统计学意义(P<0.05)。ROC曲线对LDH、LRG1、β2-MG 3种血清标记物进行分析,并预测其复发风险的最佳截断值,分别为LDH 402.37 U/L、LRG11.81 mg/L和β2-MG 168.3 ng/L。进一步的COX多因素回归分析显示,血清LRG1是影响弥漫大B细胞淋巴瘤的复发的独立因素(P<0.05)。结论:血清中LRG1的表达可能在预测弥漫大B细胞淋巴瘤复发风险中具有一定的临床意义。Objective:To investigate the clinical significance of serum LRG1,LDH andβ2-MG in the recurrence of diffuse large B-cell lymphoma(DLBCL)after chemotherapy.Methods:The serum levels of LRG1,LDH andβ2-MG of 80 patients with DLBCL were detected before treatment and followed up for these patients was performed.The cut-off value of non-recurrent survival was determined by ROC analysis.The correlation of three serum markers for predicting recurrence with prognostic factors of diffuse large B-cell lymphoma patients after treatment was analyzed by ROC curve.Results:The serum levels of LDH,LRG1 andβ2-MG were higher in the groups with high tumor stageing,extranodal invasion and bone marrow involvement,respectively(P<0.05).The optimal cut-off values for predicting recurrence risk determined by ROC analysis:LDH 402.37 U/L,LRG11.81 mg/L andβ2-MG 168.3 ng/L,respectively.COX multivariate regression analysis showed that serum LRG1 was an independent factor affecting the recurrence of diffuse large B-cell lymphoma(P<0.05).Conclusion:The serum level of LRG1 may become a new biological marker to predict the recurrence risk of diffuse large B-cell lymphoma.
