摘　　要：目的分析原发性胃癌组织中survivin基因表达情况及其与奥沙利铂耐药性的关系。方法经病理确诊且行外科手术切除的80例胃癌患者,其中有36例取得距离癌组织边界>3cm的癌旁组织标本。均采用免疫组化染色SP法分析survivin基因表达情况,通过MTT药物敏感试验分析胃癌对奥沙利铂的耐药性,并进行统计学分析。结果Survivin阳性着色位于细胞质,原发性胃癌标本中survivin基因阳性表达率明显高于癌旁组织中阳性表达率(68.75%vs11.11%),差异有统计学意义(P<0.05)。Ⅲ~Ⅳ期、发生淋巴结转移的原发性胃癌患者癌组织survivin基因阳性表达率明显高于Ⅰ~Ⅱ期、未发生淋巴结转移者,比较有统计学意义(P<0.05)。80例原发性胃癌标本对奥沙利铂耐药者22例,达27.50%,随着survivin基因表达强度的增强,奥沙利铂耐药率逐渐增高,敏感性逐渐下降,差异经秩和检验有统计学意义(P<0.05)。Spearman相关性分析显示原发性胃癌survivin基因表达与奥沙利铂耐药率呈正相关(P<0.05)。结论胃癌组织中survivin基因表达水平可反映其病理特征,且与奥沙利铂耐药性密切相关。Objective To analyze the expression of survivin gene in primary gastric cancer and its relationship with oxaliplatin resistance.Methods Of the 80 patients with gastric cancer who were pathologically confirmed and surgically resected,36 of them obtained para-cancerous tissue specimens more than 3 cm away from the margin of the cancer tissue.The expression of survivin gene was analyzed by immunohistochemical staining SP method.The drug resistance of gastric cancer to oxaliplatin was analyzed by MTT drug sensitivity test and statistical analysis was performed.Results Survivin positive staining was located in the cytoplasm.The positive expression rate of survivin gene in primary gastric cancer was significantly higher than that of adjacent tissues(68.75%vs 11.11%)(P<0.05).The positive expression rates of survivin gene in patients with stageⅢ~Ⅳand lymph node metastasis were significantly higher than those of patients with stageⅠ~Ⅱand patients without lymph node metastasis(P<0.05).There were 22 patients with oxaliplatin resistance,accounting for 27.50%.With the increase of survivin gene expression intensity,the resistance rate of oxaliplatin increased gradually,and the sensitivity decreased gradually.The rank sum test showed statistically significant differences(P<0.05).Spearman correlation analysis showed that the expression of survivin gene in primary gastric cancer was positively correlated with oxaliplatin resistance rate(P<0.05).Conclusion The expression level of survivin gene in gastric cancer tissues can reflect the pathological features and it is closely related to oxaliplatin resistance.
