恶性胸腺瘤的外科治疗效果观察

摘　　要：目的探讨恶性胸腺瘤的手术治疗效果。方法回顾分析手术治疗并经术后病理证实的恶性胸腺瘤58例,其中合并重症肌无力49例(43例眼肌型)。52例行完全性切除,6例行肿瘤+胸腺全切除。结果围手术期无死亡病例。术后48例辅助放疗,2例辅助化疗。49例获得随访,Ⅱ期5年生存率76.3%(29/38);Ⅲ期5年生存率66.7%(12/18);Ⅳ期5年生存率0%。结论恶性胸腺瘤首选手术切除,手术应尽可能做到纵隔脂肪淋巴组织的清扫。Objective To investigate the surgical treatment effect of malignant thymoma.Methods 58 cases of malignant thymoma treated surgically and confirmed by postoperative pathology were retrospectively analyzed,including 49 cases of myasthenia gravis(43 cases of ocular muscle type).Results There were no death cases in perioperative period,48 cases received adjuvant radiotherapy and 2 cases received adjuvant chemotherapy after operation.52 cases underwent complete resection,6 cases underwent tumor+thymectomy.49 cases were followed up,and the 5-year survival rate in phaseⅡwas 76.3%(29/38).The 5-year survival rate of stageⅢwas 66.7%(12/18).The 5-year survival rate of stageⅣwas 0%.Conclusion Surgical resection is the first choice for malignant thymoma.Mediastinal fat and lymph tissue should be cleaned as much as possible.
