胃癌根治术中右美托咪定辅助全麻对患者围手术期疼痛及肺功能的影响

摘　　要：目的探讨胃癌根治术中右美托咪定辅助全麻对患者围手术期疼痛及肺功能的影响。方法选择接受胃癌根治术治疗的60例胃癌患者作为研究对象,按随机数字表法分为对照组和观察组,每组30例。对照组采用常规全身诱导麻醉,观察组在对照组基础上联合右美托咪定辅助全麻,比较两组麻醉质量、麻醉用药量、不同时间点前列腺素E2(PGE2)、P物质(SP)、视觉模拟评分(VAS)、肺功能及不良反应发生情况。结果两组呼吸恢复时间、苏醒时间及拔管时间比较无统计学差异(P>0.05),观察组瑞芬太尼及丙泊酚用量低于对照组,差异有统计学意义(P<0.05)。术前两组PGE2、SP、肺功能比较无统计学差异(P>0.05);术后两组PGE2、SP、RV/TLC均上升,(MVV-VE)/FEV1、MVV/FEV1均下降,观察组变化幅度更小(P<0.05)。两组总不良反应发生率比较无统计学差异(P>0.05)。结论右美托咪定辅助全麻可减轻胃癌根治术患者围手术期疼痛,保护肺功能,不影响患者苏醒时间。Objective To investigate the influences of dexmedetomidine assisted general anesthesia in radical gastrectomy on perioperative pain and lung function.Methods 60 cases of gastric cancer patients who received radical gastrectomy were selected as the research objects,and according to the random number table method those patients were divided into the control group and the observation group,30 cases in each group.Control group using conventional whole body induction of anesthesia,observation group in the control group on the basis of dexmedetomidine the auxiliary anesthesia,then anesthesia,anesthesia dose,different timepoints of prostaglandin E2(PGE2)and substancep(SP),visual analogue scale(VAS),pulmonary function,and adverse reactions occur in both group were compared.Results There was no statistically significant difference in respiratory recovery time,recovery time and extubation time between the 2 groups(P>0.05).The dosage of remifentanil and propofol in the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).Before surgery,there were no statistical differences in PGE2,Sp,lung function between the 2 groups(P>0.05).After surgery,PGE2,SP,RV/TLC were increased in both groups,while(MVV-VE)/FEV1 and MVV/FEV1 were decreased in both groups,the difference was statistically significant(P<0.05).There was no statistical difference in the incidence of total adverse reactions between the 2 groups(P>0.05).Conclusion Dexmedetomidine assisted general anesthesia can alleviate perioperative pain in patients undergoing radical gastrectomy,protect lung function,and does’t affect the patient’s waking time.
