腹腔镜筋膜内前列腺根治术对不同年龄前列腺癌患者控尿能力及性功能恢复的影响

摘　　要：目的探讨腹腔镜筋膜内前列腺根治术对不同年龄前列腺癌患者控尿及性功能恢复的影响。方法将62例前列腺癌患者根据患者年龄分为A组(≤60岁)及B组(>60岁),2组患者均行腹腔镜筋膜内前列腺根治术。对比2组患者的围术期指标,术后并发症发生情况,术后3、6、12个月的控尿能力以及术前、术后3、6及12个月的IIEF-5评分。结果2组患者的手术时间、术中出血量和留置尿管时间对比,无统计学差异(P>0.05);术后A组患者并发症发生率为7.14%,B组患者术后并发症发生率为11.76%,2组差异无统计学意义(P>0.05)。术后3、6个月时,A组控尿正常率显著高于B组(P<0.05);术后12个月时,2组的控尿能力对比差异无统计学意义(P>0.05)。A组术后3、6个月时的IIEF-5评分显著低于术前(P<0.05),术后12个月时与术前对比差异无统计学意义(P>0.05);B组术后3、6、12个月的IIEF-5评分均明显低于术前;术前、术后3个月2组的IIEF-5评分对比差异无统计学意义(P>0.05),术后6、12个月时A组的IIEF-5评分明显高于B组(P<0.05)。结论腹腔镜筋膜内前列腺根治术对前列腺癌患者均安全有效,术后年轻患者的控尿能力及性功能恢复较快,该手术对年轻患者的治疗效果更佳。Objective To explore the effects of laparoscopic intrafascial prostatectomy on urinary control and sexual function recovery in patients with prostate cancer of different ages.Methods 62 cases of prostate cancer were selected and divided into group A(no more than 60 years old)and group B(>60 years old)according to the age of the patients.Both groups underwent laparoscopic intrafusalprostatectomy.Theperioperative indicators,the incidence of postoperative complications,the ability to control urine at 3,6 and 12 months after surgery and the IIEF-5 scores at 3,6 and 12 months after surgery were compared between the 2 groups.Results There was no significant difference in operation time,intraoperative blood loss and time of indwelling urethral catheter between the two groups,P>0.05.The complication rate of group A after operation was 7.14%.The complication rate of group B after operation was 11.76%.There was no statistically significant difference between the 2 groups,P>0.05.3 and 6 months after surgery,the normal rate of urinary control in group A was significantly higher group B,P<0.05.12 months after surgery,there was no statistically significant difference in urinary control ability between the 2 groups,P>0.05.3 and 6 months after surgery,the IIEF-5 scores of group A were significantly lower thanpreoperative,there were no statistical significance between 12 months after surgery andpreoperative.3 and 6 months after surgery,the IIEF-5 scores of group B were significantly lower thanpreoperative,there were no statistical significance between 3 months after surgery andpreoperative.6 and 12 months after operation,the IIEF-5 scores in group A was significantly higher than group B.Conclusion The control ability and sexual function of young patients after laparoscopic radical prostatectomy recovered quickly,and the treatment effect of this surgery is better for young patients.
