摘 要:目的探讨脑脊液生物标志物在早期帕金森病(Parkinson’s disease,PD)诊断中的临床应用价值。方法选取PD患者170例,依据H-Y评分分为早期组(90例)、中期组(60例)和晚期组(20例);另选取健康志愿者30例为对照组。采集所有受试者脑脊液标本,免疫比浊法检测β-内啡肽(β-EP)及亮氨酸-脑啡肽(LEK),酶联免疫吸附法检测α-突触核蛋白(α-syn),并进行组间比较。同时,分析H-Y评分与β-EP、LEK及α-syn水平的相关性。结果早、中、晚期3组PD患者脑脊液中β-EP水平分别为(42.74±2.37)pg/mL、(39.12±3.92)pg/mL、(36.64±3.86)pg/mL,而LEK水平分别为(91.57±4.25)、(92.26±5.04)、(92.37±5.41)ng/L;PD患者脑脊液中β-EP水平显著低于对照组[(46.21±1.37)pg/mL],而LEK水平显著高于对照组[(86.05±2.58) ng/L],差异均有统计学意义(P<0.05)。PD患者脑脊液α-syn水平显著低于对照组,差异有统计学意义(P<0.05)。PD患者脑脊液中β-EP、α-syn水平与疾病分期具有相关性(r=-0.632,-0.742,P<0.05)。结论通过检测脑脊液中β-EP、α-syn水平有助于早期诊断PD并评估病情。Objective To investigate the clinical value of cerebrospinal fluid(CSF) biomarkers in the diagnosis of early Parkinson’s disease(PD).Methods 170 patients with PD were selected and were divided into early-stage group(90 cases),middle-stage group(60 cases) and late-stage group(20 cases) according to H-Y score.Another 30 healthy volunteers were selected as control group.The CSF samples were collected of all subjects,and the levels of β-EP and LEK were tested by immunoturbidimetry,α-syn was tested by ELISA.The correlation between H-Y score and levels of beta-EP,LEK and alpha-syn was analyzed.Results The levels of β-EP in CSF of early,middle and late-stage PD patients were(42.74±2.37)pg/mL,(39.12±3.92)pg/mL,(36.64±3.86)pg/mL,respectively.The LEK levels were(91.57±4.25),(92.26±5.04),(92.37±5.41)ng/L,respectively.The level of β-EP in CSF of PD patients was significantly lower than that of control group[(46.21±1.37)pg/mL],LEK level was significantly higher than that of control group[(86.05±2.58) ng/L],the difference was statistically significant(P<0.05).The level of CSF α-syn in PD patients was significantly lower than that in control group(P<0.05).The levels of β-EP and α-syn in CSF of PD patients were correlated with the stage of disease(r=-0.632,-0.742,P<0.05).Conclusion The levels of β-EP and α-syn in CSF is helpful for early diagnosis and assessment of PD.
