铜川地区3019例女性HPV感染状况及基因分型调查

摘　　要：目的探讨铜川地区女性人乳头瘤病毒(HPV)感染率、基因型和年龄分布特征,确定本地区的HPV优势型。方法选择2014年3月至2016年8月在本院妇产科就诊的3 019例门诊及住院女性为研究对象,采用PCR-反向点杂交法进行基因型检测,分析HPV阳性患者的基因型分布情况及患者的年龄分布情况。结果 3 019例标本中检出HPV阳性690例,阳性率为22.86%,高危型、疑似高危型及低危型感染阳性率分别为67.83%、7.54%、24.64%。高危型以HPV16型的阳性率最高,其次为HPV52型、HPV58型。低危型以HPV44型阳性率最高,其次为HPV42型、HPV40型。本地区HPV感染以单一型为主,占68.55%,二重及多重感染占15.65%。HPV感染阳性人群主要集中在36~55岁年龄段。结论本地区女性HPV感染率为22.86%,优势型为HPV16、52、58、53、44型,筛查工作应重点关注36~55岁的妇女,对其定期进行HPV筛查,针对性应用预防性疫苗。Objective To study the infection rate, genotype and age distribution of women with human papillomavirus(HPV) in Tongchuan, and to determine the dominant type of HPV in this area. Methods From March 2014 to August 2016,3 019 women in outpatients and inpatients from gynecology and obstetrics department of our hospital were selected as the research objects. The genotypes were detected by PCR reverse dot hybridization method, and the genotypes and age distribution of HPV positive patients were analyzed. Results Among the 3 019 samples, 690 samples were positive for HPV,with a positive rate of 22.86%. The positive rates of high-risk, suspected high-risk and low-risk types were 67.83%,7.54% and 24.64% respectively. In high-risk type, HPV16 has the highest positive rate, followed by HPV52 and HPV58.In low-risk type, HPV44 has the highest positive rate, followed by HPV42 and HPV40. In this area, HPV infection was mainly single type, accounting for 68.55%, and double and multiple infection accounted for 15.65%. The HPV positive population mainly concentrated in the age group of 36-55. Conclusion The rate of HPV infection in women was 22.86%,and the dominant type of HPV are types 16, 52, 58, 53 and 44. The screening work should focus on the women aged 36-55, and should give them regular HPV screening and preventive vaccine specifically.
