摘 要:目的探讨非小细胞肺癌脑转移患者血清肿瘤标志物水平的变化及临床意义。方法回顾性分析2015-01-2017-12在郑州大学第二附属医院住院的135例非小细胞肺癌(non-small cell lung cancer,NSCLC)患者的临床资料,其中57例非小细胞肺癌脑转移患者(观察组),78例非小细胞肺癌非脑转移患者(对照组)和同期105例健康体检者(健康组),应用化学发光法检测血清肿瘤标志物甲胎蛋白(alpha-fetoprotein,AFP)、癌胚抗原(carcinoembryonic antigen,CEA)、神经元特异性烯醇化酶(neuron-specific enolase,NSE)、糖链抗原125(sugar chain antigen,CA-125)、鳞状细胞癌抗原(squamous cell carcinoma antigen,SCC-Ag)和细胞角蛋白片段(cytokeratin fragments,CYFRA21-1)水平变化,并进行比较。结果 NSCLC患者肿瘤标志物AFP、CEA、SCC-Ag和CYFRA21-1含量显著高于健康组(P<0.05),观察组血清肿瘤标志物高于对照组(P<0.05)。观察组和对照组性别、年龄、吸烟史、AFP、NSE、CA125和病理类型差异均无统计学意义(P>0.05),观察组CEA、SCC-Ag、CYFRA21-1水平和肿瘤细胞低分化程度高于对照组,差异有统计学意义(P<0.05)。结论血清肿瘤标志物CEA、SCC-Ag、CYFRA21-1水平和肿瘤细胞分化程度可作为NSCLC脑转移的独立危险因素,可能对NSCLC脑转移患者的诊断、预后评估具有一定的临床意义。Objective To evaluate the correlation between brain metastasis and serum tumor markers in patients with non-small cell lung cancer(NSCLC).Methods The clinical data of 135 cases of NSCLC were analyzed retrospectively,included 57 cases of brain metastasis(study group),78 cases without brain metastasis(control group) and 105 cases of healthy persons(healthy group) were selected in the same period.The lung tumor markers of AFP,CEA,NSE,CA125.SCC-Ag and CYFRA21-1 were determined by chemiluminescence.The determination levels of serum tumor markers were compared between 3 groups.Results The lung tumor markers CEA,SCC-Ag and CYFRA21-1 levels of study group were higher than control group(P<0.05)and it was not related to gender,age,smoking history,AFP,NSE,CA125 and pathological type(P>0.05),and CEA,SCC-Ag and CYFRA21-1 increased,the degree of differentiation were statistical significance(P<0.05).Conclusion The serum tumor markers CEA,SCC-Ag,CYFRA21-1 and the degree of differentiation are independent risk factors for brain metastasis of NSCLC and improve the diagnosis evaluation of treatment for NSCLC with brain metastasis.
