摘　　要：目的 探讨血清热休克蛋白90α(HSP90α)在胆管癌诊断中的价值。方法 选取2012年9月至2017年10月中国人民解放军第二五一医院收治的胆管癌住院患者54例(胆管癌组),另选取同期住院胆管结石患者32例(胆管结石组),同期行健康体检者25例(对照组)。用酶联免疫吸附试验(ELISA)检测所有受试者血清HSP90α水平,并以HSP90α>82.06ng/mL为阳性判定标准,分析胆管癌患者血清HSP90α阳性率与临床参数的关系。结果 胆管癌组血清HSP90α水平[(104.53±23.21)ng/mL]高于胆管结石组[(42.53±8.86)ng/mL]及对照组[(38.22±9.85)ng/mL],差异均有统计学意义(P<0.05);胆管结石组与对照组血清HSP90α水平无明显差异(P>0.05)。胆管癌患者血清HSP90α阳性率与肿瘤分化程度、淋巴结转移、临床TNM分期有关(P<0.05)。结论 HSP90α在胆管癌患者血清中高表达,与肿瘤的恶性进展密切相关。Objective To investigate the value of serum heat shock protein 90α(HSP90α)in the diagnosis of cholangiocarcinoma.Methods A total of 54 cases of patients with cholangiocarcinoma admitted to the 251st Hospital of PLA from September 2012 to October 2017 were enrolled(the cholangiocarcinoma group),and other 32 cases of patients with bile duct stones(the biliary stone group)and 25 healthy individuals received health examination(the control group)in the same period were selected.The serum levels of HSP90αwere measured by ELISA.HSP90α>82.06 ng/mL was positive decision criteria,and the relationships between serum HSP90αpositive rate and clinical parameters in patients with cholangiocarcinoma were analysed.Results The serum HSP90αlevel in the cholangiocarcinoma group[(104.53±23.21)ng/mL]was higher than that in the biliary stone group[(42.53±8.86)ng/mL]and the control group[(38.22±9.85)ng/mL],there were statistically significant differences(P<0.05).No significant difference was found in the serum HSP90αlevel between the biliary stone group and the control group(P>0.05).The positive rate of serum HSP90αwas related to tumor differentiation,lymph node metastasis and clinical TNM staging in patients with cholangiocarcinoma(P<0.05).Conclusion The serum HSP90αis highly expressed in patients with cholangiocarcinoma,which might be closely related to the malignant progression of tumors.
