从“虚-郁-滞”病机浅谈肝癌的发病机理

摘　　要：肝癌是由多因素综合作用而引起的临床常见恶性肿瘤,发病机理尚未完全清楚。多数医家认为其病机多为肝脾两虚、外来邪毒、七情内伤、饮食不节、湿热内蕴等。临床上肝癌患者多见肝郁脾虚,肝郁内伤所致气机郁滞进而产生瘀、痰等邪毒。此外,外来邪气积聚体内,瘀久成毒,"痰浊、瘀血、邪毒"留滞又促进郁结,郁滞不解加重脾虚,最后导致"虚、郁、滞"三者互结,恶性循环。因此,如何从"虚、郁、滞"为切入点,阐明肝癌的中医发病机理值得深入探讨。本文在分析肝癌病因病机的基础上,认为正气内虚为肝癌发病之本,气郁不畅、邪毒留滞为肝癌的促病之源,阐述了"虚-郁-滞"病机在肝癌发病中的"发病之本"与"促病之源"的关系,将肝癌的病机化繁为简,为肝癌的辨治提供可行的临床思路。Hepatocellular carcinoma(HCC),which is caused by multiple factors,is a common clinical malignant tumor.Its pathogenesis is unclear.Most doctors think that the cause of the disease is mainly the deficiency of liver and spleen,external evil qi,internal injury of seven emotions,improper diet,damp heat,etc.In clinical,HCC is mostly characterized by stagnation of liver qi and spleen.The stagnation of qi caused by internal injury of liver depression,which leads to blood stasis and phlegm.In addition,external evil qi accumulates in the body and accumulates for a long time to form poison.Stagnation of phlegm,blood stasis and evil toxin promote stagnation,then the stagnation aggravates the spleen deficiency,and then“deficiency,depression and stagnation”are combined to lead into a vicious circle.Therefore,how to clarify the pathogenesis of liver cancer from the point of“deficiency,depression and stagnation”is worthy researching.Based on the analysis of the cause of diseases and pathogenesis of liver cancer,this paper holds that the deficiency of qi and internal qi is the root cause of HCC,the stagnation of qi and toxin is the source of promoting HCC,and expounds the relationship between the pathogenesis of“deficiency,depression and stagnation”and the sources of promoting HCC in the process of HCC,so as to simplify illuminate the pathogenesis of HCC and provide some clinical ideas for the diagnosis and therapy of HCC.
