内镜黏膜下剥离术在胃肠黏膜病变中的应用效果观察

摘　　要：目的:探讨分析内镜黏膜下剥离术在胃肠黏膜病变中的应用效果.方法:选取我院内镜室2016年9月-2018年9月收治的142例胃肠膜病变患者作为对象,所有患者采用内镜黏膜下剥离术治疗,观察其术后治疗效果以及并发症发生情况.结果:内镜黏膜下剥离术完成率100%;并发病发生:穿孔8例,术中出血33例,延迟性出血8例.142例患者手术中共切除156处病灶,其中胃部134处,一次性整块切除129处;直肠22处,一次性整块切除19处.治愈性切除143处,胃部129处,直肠14处.非治愈性切除13处,胃部10处,直肠3处.142例患者的术后病理为高级别上皮内癌变79例,低级别上皮内瘤变32例,增生性息肉19例,早癌6例,异位胰腺6例.结论:内镜黏膜下剥离术对胃肠黏膜病变有着较好的治疗效果,能有效减少并发症的发生,安全性高,具有较高临床应用价值.Objective To explore and analyze the nursing effect of endoscopic submucosal dissection in gastrointestinal mucosal lesions.Methods142 patients with gastrointestinal lesions admitted to the endoscopy room of our hospital from September 2016 to September 2018 were selected as the study subjects.All patients were treated with endoscopic submucosal dissection.The therapeutic effect and complications were observed.Results The complete rate of endoscopic submucosal dissection was 100%.There were 20 cases of perforation,33 cases of intraoperative bleeding and 8 cases of delayed bleeding.In 142 cases,156 lesions were resected,including 134 cases of stomach,129 cases of one-time whole resection,22 cases of rectum,19 cases of one-time whole resection.Curative resection was performed in 143 cases,including 129 cases of stomach and 14 cases of rectum.Non-curable resection was performed in 13 cases,10 in the stomach and 3 in the rectum.Postoperative pathology of 142 patients included high-grade intraepithelial carcinogenesis in 79 cases,low-grade intraepithelial neoplasia in 32 cases,proliferative polyps in 19 cases,early cancer in 6 cases and heterotopic pancreas in 6 cases.Conclusion Endoscopic submucosal dissection has a good therapeutic effect on gastrointestinal mucosal lesions.It can effectively reduce the occurrence of complications and has high safety.It has clinical application and popularization value.
