摘　　要：目的了解杭州市实施适龄儿童第一恒磨牙免费窝沟封闭项目的防龋效果。方法抽取接受第一恒磨牙窝沟封闭5年后的儿童504名进行龋患和封闭剂保留情况检查,同时选取当年检查时第一恒磨牙未接受封闭的儿童作为对照。结果试验组第一恒磨牙窝沟封闭5年后龋患率为11.83%,封闭剂保留率39.43%,患龋的牙数随着窝沟封闭剂保留率的下降而增多,未封闭对照组的第一恒磨牙龋患率为29.22%,龋降低相对有效率为49.81%,龋降低实际有效率为11.74%。女生龋降低实际有效率17.23%,高于男生(6.80%)。农村儿童龋降低实际有效率13.79%,高于城市(8.77%)。结论窝沟封闭术能明显降低第一恒磨牙龋病的发病率,特别是对于女生和农村地区的儿童,是适宜推广的预防和控制龋病的技术。Objective To investigate the outcomes of caries prevention using pit and fissure sealant for the first permanent molars among school-age children in Hangzhou city.Methods Examinations for sealant retention and caries were carried out in a randomly selected group of 504 pupils who received pit and fissure sealant for their first permanent molars five years ago,and also in a contemporary control group of age-matched pupils who did not received pit and fissure sealant for the first permanent molars.Results Among the pupils having the pit and fissure sealant 5 years ago,the detection rate of caries was 11.83%,the sealant retention rate was 39.43%,and the number of teeth with caries increased with less retention of the sealant.In control pupils who had not received pit and fissure sealant,the detection rate of cares was 29.22%.Overall,the relative effectiveness rate of dental caries reduction was 49.81%,and the actual effectiveness rate of dental caries reduction was 11.74%.The actual effectiveness rate of dental caries reduction was higher in females(17.23%)than in males(6.80%),and in of children from rural areas(13.79%)than in those from urban areas(8.77%).Conclusion Pit and fissure sealant for the first permanent molars is effective for prevention of dental caries among school-age children,especially among females and children fromrural areas,therefore justified for widespread use as a proper measure against caries.
