摘　　要：目的 探讨Solitaire AB支架取栓术联合多模式血管再通术治疗急性脑梗死的有效性及安全性.方法 回顾性分析2018年03月-2019年05月采用Solitaire AB支架取栓术联合多模式血管再通术治疗14 例急性脑梗死患者的临床病例资料,分析Solitaire AB支架取栓联合多模式血管再通术治疗急性脑梗死的有效性及安全性.结果 该组14 例患者术中血管再通血流灌注达到mTICI 3级7 例(50.0%),mTICI 2b级5 例(35.7%),血管再通良好率85.7%.术后复查头颅CT,发生高灌注出血1 例,高灌注脑水肿脑疝1 例,2 例患者均死亡.出血转化4 例,其中症状性颅内出血2 例.术后90 d随访,预后良好10 例(mRS评分≤2 分)(71.4%),预后不良4 例(mRS评分>2 分)(28.6%),包括死亡2 例.结论 Solitaire AB支架取栓联合多模式血管再通术治疗急性脑梗死,可显著提高患者的血管再通率,改善患者远期预后,提高患者生存质量,安全有效.Objective To investigate the efficacy and safety of Solitaire AB stent combined with multi-mode thrombectomy on recanalization in patients with acute cerebral infarction (ACI) and large vessel occlusion (LVO). Methods A total of 14 ACI patients with LVO who underwent Solitaire AB stent combined with multi-mode thrombectomy were enrolled from March 2018 to May 2019 in Suzhou Xiangcheng People's Hospital were included in the present study. Results After thrombectomy treatment, 7 patients (50.0%) with successful revascularization as modified Thrombolysis in Cerebral Infarction (mTICI) score of 3, 5 patients (35.7%) with successful revascularization as mTICI score of 2b, the successful revascularization was 85.7%. 2 patients died of intracerebral hemorrhage and cerebral edema by hyperperfusion after recanalization. There were 4 patients with hemorrhagic transformation during the follow-up, including 2 symptomatic intracranial hemorrhage. 10 patients (71.4%) showed good functional outcome defined as modified Rankin Scale (mRS) ≤ 2 and 4 patients with poor functional outcome defined as mRS >2, including 2 patients who died during the 90 days follow-up. Conclusion Among patients with ACI and LVO, Solitaire AB stent combined with multi-mode thrombectomy treatment were effective and safe with high rate of recanalization and good functional outcome.
