化湿通络方灌肠联合美沙拉嗪肠溶片治疗溃疡性结肠炎临床研究

摘　　要：目的:观察化湿通络方灌肠联合美沙拉嗪肠溶片治疗溃疡性结肠炎的临床疗效。方法:将124例溃疡性结肠炎大肠湿热证患者分为对照组和观察组各62例。2组均以美沙拉嗪肠溶片治疗,观察组加用化湿通络方灌肠,2组疗程均为4周。治疗前后评定腹痛、腹泻、血便和里急后重评分,检测炎症因子肿瘤坏死因子-α(TNF-α)、白细胞介素-5(IL-5)和白细胞介素-6(IL-6)的水平。比较2组的临床疗效和不良反应发生情况。结果:观察组总有效率93.6%,对照组总有效率74.2%,2组比较,差异有统计学意义(P<0.05)。2组腹痛、腹泻、血便和里急后重评分均较治疗前下降(P<0.05),观察组4项症状评分均低于对照组(P<0.05)。2组血清炎症因子TNF-α、IL-5和IL-6水平均较治疗前下降(P<0.05),观察组TNF-α、IL-5和IL-6水平均低于对照组(P<0.05)。2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:在美沙拉嗪肠溶片治疗的基础上加用化湿通络方灌肠治疗溃疡性结肠炎大肠湿热证患者,能有效减轻症状、抑制炎症因子表达,提高临床疗效,安全性高。Objective:To observe the clinical effect of enema with Huashi Tongluo prescription combined with mesalazine enteric-coated tablets for ulcerative colitis.Methods:A total of 124 cases of patients with ulcerative colitis with large intestinal dampness-heat syndrome were randomly divided into the control group and the observation group,62 cases in each group.Both groups were treated with mesalazine enteric-coated tablets,and the observation group was additionally treated with enema with Huashi Tongluo prescription.Both groups were treated for four weeks.Before and after treatment,the scores of abdominal pain,diarrhea,bloody stool and tenesmus were evaluated,and the levels of inflammatory factors including tumor necrosis factor-a(TNF-a),interleukin-5(IL-5)and interleukin-6(IL-6)were detected.The clinical effect and the incidence of adverse reactions in the two groups were compared.Results:The total effective rate was 93.6%in the observation group and 74.2%in the control group,the difference being significant(P<0.05).After treatment,the scores of abdominal pain,diarrhea,bloody stool and tenesmus in the two groups were decreased when compared with those before treatment(P<0.05),and the four symptom scores in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of inflammatory factors in serum including TNF-a,IL-5 and IL-6 in the two groups were decreased when compared with those before treatment(P<0.05).The levels of TNF-a,IL-5 and IL-6 in the observation group were lower than those in the control group(P<0.05).There was no significant difference being found in the comparison of the incidence of adverse reactions in the two groups(P>0.05).Conclusion:The additional application of enema with Huashi Tongluo prescription based on mesalazine enteric-coated tablets for ulcerative colitis with large intestinal dampness-heat syndrome can effectively alleviate symptoms,inhibit the expressions of inflammatory factors and improve the clinical effect with high safety.
