保肝降脂汤联合水飞蓟宾治疗非酒精性脂肪性肝炎临床研究

摘　　要：目的:观察在常规疗法基础上以保肝降脂汤联合水飞蓟宾治疗非酒精性脂肪性肝炎的疗效。方法:选择本院收治的108例肝郁脾虚型非酒精性脂肪性肝炎患者作为观察对象,随机分为观察组和对照组各54例,2组患者均接受常规治疗,对照组给予水飞蓟宾胶囊治疗,观察组给予保肝降脂汤联合水飞蓟宾胶囊治疗,2组均治疗12周。观察2组治疗前后的血脂[总胆固醇(TC)、甘油三酯(TG)]、氧化应激[丙二醛(MDA)、超氧化物歧化酶(SOD)]、炎症因子[白细胞介素-1(IL-1)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]及肝功能[谷丙转氨酶(ALT)、谷草转氨酶(AST)、碱性磷酸酶(ALP)、γ-谷氨酰基转移酶(γ-GT)]指标的变化情况,比较2组治疗后的疗效。结果:治疗后,观察组总有效率达92.59%,高于对照组的77.78%,差异有统计学意义(P<0.05)。2组TC、TG、MDA、IL-1、IL-6、TNF-α、ALT、AST、ALP及γ-GT水平均较治疗前降低,SOD水平均较治疗前升高,差异均有统计学意义(P<0.05);观察组TC、TG、MDA、IL-1、IL-6、TNF-α、ALT、AST、ALP及γ-GT水平均低于对照组,SOD水平高于对照组,差异均有统计学意义(P<0.05)。结论:在常规疗法基础上以保肝降脂汤联合水飞蓟宾胶囊治疗非酒精性脂肪性肝炎疗效确切,可明显降低患者的血脂、炎症因子水平,改善氧化应激及肝功能。Objective:To observe the curative effect of Baogan Jiangzhi tang combined with Silibinin based on the routine treatment for nonalcoholic steatohepatitis.Methods:A total of 108 cases of patients with nonalcoholic steatohepatitis of liver depression and spleen deficiency type in our hospital were selected as the study subjects and were randomly divided into the observation group and the control group,54 cases in each group.Both groups received routine treatment;the control group was additionally treated with Silibinin capsules,and the observation group was additionally treated with Baogan Jiangzhi tang based on the treatment of the control group.Both groups were treated for twelve weeks.Before and after treatment,the changes of indexes of the blood liquids including total cholesterol(TC)and triglyceride(TG),the oxidative stress including malondialdehyde(MDA)and superoxide dismutase(SOD),the inflammatory factors including interleukin-1(IL-1),interleukin-6(IL-6)and tumor necrosis factor-a(TNF-a),as well as the liver function including alanine aminotransferase(ALT),aspartate aminotransferase(AST),alkaline phosphatase(ALP)andγ-glutamyltransferase(y-GT)in the two groups were observed.After treatment,the curative effect in the two groups was compared.Results:After treatment,the total effective rate was 92.59%in the observation group,higher than that of 77.78%in the control group,the difference being significant(P<0.05).After treatment,the levels of TC,TG,MDA,IL-1,IL-6,TNF-a,ALT,AST,ALP andγ-GT in the two groups were decreased when compared with those before treatment,while the levels of SOD were increased when compared with those before treatment,differences being significant(P<0.05);the levels of TC,TG,MDA,IL-1,IL-6,TNF-a,ALT,AST,ALP andγ-GT in the observation group were lower than those in the control group,while the level of SOD was higher than that in the control group,differences being significant(P<0.05).Conclusion:The therapy of Baogan Jiangzhi tang combined with Silibinin capsules based on the routine treatmen
