肺岩宁方治疗晚期非小细胞肺癌临床研究

摘　　要：目的:探究肺岩宁方对晚期非小细胞肺癌患者血清调节性T细胞CD4+和CD25+的影响。方法:将晚期非小细胞肺癌患者83例,按随机数字表法分为2组,对照组41例予以常规化疗,观察组42例予以肺岩宁方进行治疗。观察2组患者治疗前后中医证候、免疫功能指标、血管内皮生长因子(VEGF)、血清CYFRA21-1和生存质量(KPS)评分变化情况。结果:治疗后,观察组食欲不振、腰膝酸软、少气懒言、神疲乏力、动则气喘等症状改善情况优于对照组,差异有统计学意义(P<0.05)。观察组治疗后CD3^+、CD4^+/CD8^+、NK和CD4^+/CD25^+水平优于对照组,差异有统计学意义(P<0.05)。观察组治疗后血清VEGF、CYFRA21-1及KPS评分优于对照组,差异有统计学意义(P<0.05)。结论:肺岩宁方可以有效改善晚期非小细胞肺癌患者的中医临床症状、提高机体对血清CD4^+和CD4^+CD25^+等T细胞亚群水平的调节能力,改善免疫功能,降低VEGF和血清CYFRA21-1水平,提高生存质量。Objective:To explore the effect of Feiyanning prescription on regulatory T cells CD4^+and CD25^+in serum in patients with advanced non-small cell lung cancer.Methods:A total of 83 patients with advanced non-small cell lung cancer were divided into two groups according to the random number table method.The 41 cases in the control group received conventional chemotherapy,and42 patients in the observation group were treated with Feiyanning prescription.The changes of Chinese medicine syndromes,immune function indexes,vascular endothelial growth factor(VEGF),CYFRA21-1 in serum and the scores of karnofsky performance status scale(KPS)in the two groups were observed before and after treatment.Results:After treatment,the improvement of symptoms such as loss of appetite,weak waist and knee,weak breathing and laziness to speak,spiritlessness and weakness,and asthma once they moved was better in the observation group than that in the control group,the difference being significant(P<0.05).After treatment,the levels of CD3^+,CD4^+/CD8^+,NK and CD4^+/CD25^+in the observation group were superior to those in the control group,the difference being significant(P<0.05).After treatment,the levels of VEGF and CYFRA21-1 in serum and the KPS scores in the observation group were better than those in the control group,the difference being significant(P<0.05).Conclusion:Feiyanning prescription can effectively improve the clinical symptoms of Chinese medicine syndrome in patients with advanced non-small cell lung cancer,improve their ability to regulate the levels of T cell subgroups,namely,CD4^+and CD4^+CD25^+in serum,improve immune function,reduce the levels of VEGF and CYFRA21-1 in serum,and improve the quality of life.
