白内障术中角膜后弹力层脱离前房注气术治疗的临床观察

摘　　要：目的观察白内障超声乳化联合人工晶状体植入术致角膜后弹力层脱离前房注气术治疗的临床效果。方法回顾性分析2010年1月至2014年12月于固始县人民医院行白内障超声乳化联合人工晶状体植入术致角膜后弹力层脱离通过前房注气术治疗的患者13例(13只眼),其中大于1/2角膜面积的广泛后弹力层脱离1例(伴有部分后弹力层缺失),大于1/4并小于1/2角膜面积的后弹力层脱离3例,小于1/4角膜面积的后弹力层脱离9例。所有患者均于术毕前前房注人无菌空气,利用气泡顶压脱离的后弹力层复位;后弹力层复位不良或空气吸收后仍未复位者,以同样方法再次注气。结果前房内空气于3~5d吸收,10例患者经1次注气角膜恢复透明;2例经2次注气,1例经3次注气,均为脱离范围大于1/4角膜面积的患者;伴有后弹力层缺失的1例患者恢复较慢,长达3个月,角膜恢复透明。结论前方注气治疗白内障术中角膜后弹力层脱离,方法简单易行,可以重复操作,临床疗效确切。Objective To observe the clinical effect of intracameral air injection treating Descemet membrane detachment caused by Phacoemulsification and intraocular lens(IOL)implantation.Methods Through retrospective analysis,Phacoemulsification and IOL implantation leading to 13 cases"Descemet membrane detachment which was cured by intracameral air injection,with one case having more than half of the cormneal area's extensive Descemet membrane detachment(containing partially missing Descemet membrane),3 cases having a quarter to half of it,and 9 cases having less than a quarter of it from January 2010 to December 2014.All patients"anterior chamber was injected sterile air to reset Descemet membrane detachment before the end of surgery.If the approach doesn't work,the patients can be injected again.Results The air in the anterior chamber was absorbed within 3 to 5 days after surgery.Ten cases"corneas recovered transparently by one injection.Moreover,3 cases were greater than a quarter of corneal area of Descemet membrane detachment,2 of them recovered by twice injections,and one of them recovered by three injections.In addition,one case recovered transparently until 3 months later because of partially missing Descemet membrane.Conclusions Intracameral air injection treating Descemet membrane detachment in cataract surgery is easily,repeatedly,and efectively.
