摘　　要：目的应用彩色多普勒超声,研究高眼压的原发性闭角型青光眼患者行小梁切除术降低眼压后眼底血流动力学变化。方法前瞻性研究。共48例高眼压原发性闭角型青光眼患者应用彩色多普勒超声检测其眼动脉、视网膜中央动脉及睫状后短动脉的收缩期最大血流速度、舒张末期血流速度及阻力指数。患者行小梁切除术并降低眼压后复查以上血流参数将每组数据间进行比较。手术前后分别采用Goldmann压平式眼压计测量3次眼压。结果术后降低眼压后,术眼球后血流参数结果与基线测量值比较差异无统计学意义(P>0.05)。手术前后,眼压的改变与球后血流速度参数的改变之间无显著相关性(P>0.05)。结论原发性闭角型青光眼患者小梁切除术后的眼压改变与眼底血流参数的改变之间无相关性,即使术后眼压正常情况下也不能改善其眼底血流状态。Objective To assess the retrobulbar hemodynamic parameters after decreasing elevated intraocular pressure(IOP)in patients with primary angle-closure glaucoma(PACG)by using color Doppler imaging.Methods A total of 48 eyes from 48 patients with PACG,with elevated IOP,were consecutively included in this prospective study.Peak-systolic velocity,end-diastolic velocity,and resistivity index were assessed in the OA,CRA,and PCA by using Color Doppler imaging(CDI).Reassessed the hemodynamic parameters in patients after trabeculectomy.Compared the hemodynamic parameters at baseline and after IOP reduction.The IOP was measured with Goldmann applanation tonometer(GAT),before and after the trabeculectomy,3 times respectively.Results After decreasing the elevated IOP,the retrobulbar hemodynamic parameters of the eye after trabeculectomy showed no differences as compared with baseline measurements.There was no correlation between the changes in IOP and the changes in the retrobulbar hemodynamic parameters(p>0.05 for all).Conclusions This study suggested a lack of correlation between the changes in IOP and the changes in the retrobulbar hemodynamic parameters in the patients with PACG after trabeculectomy.The blood flow disturbances won't be improved even the IOP was In the normal range after trabeculectomy.
