旋金降逆汤联合电针足三里治疗反流性食管炎(肝胃郁热证)临床研究

摘　　要：目的:评价旋金降逆汤联合电针足三里治疗反流性食管炎(肝胃郁热证)的临床疗效和探讨其可能的胃动力机制。方法:将140例反流性食管炎(肝胃郁热证)患者随机分为4组,每组35例,包括治疗1组、治疗2组、治疗3组和对照组,分别给予旋金降逆汤、电针足三里、旋金降逆汤联合电针足三里、奥美拉唑治疗,治疗时间为8周,治疗结束后比较4组临床症状、胃肠激素和内镜下食管黏膜炎症的改善情况。结果:(1)4组的临床总有效率分别为68.57%、57.14%、91.43%和71.43%,经统计学比较,治疗3组临床疗效优于对照组及其他治疗组(P<0.05);(2)4组均能有效增加血浆胃动素、Ghrelin,治疗3组优于对照组及其他治疗组(P<0.05);(3)4组均能改善食管黏膜炎症,经统计学比较,各组之间无显著差异(P>0.05)。结论:旋金降逆汤联合电针足三里可能是通过增加胃肠激素,调节胃动力,促进胃排空,从而缓解改善反流性食管炎(肝胃郁热证)临床症状,促进食管黏膜炎症愈合。Objective:To evaluate the clinical effects of Xuanjin Jiangni Decoction combined with electroacupuncture at Zusanli(ST36)on treating reflux esophagitis with the type of heat stagnation of liver and stomach,and discuss the possible mechanism of the method on gastric motility.Methods:A total of 140 patients with reflux esophagitis with the type of heat stagnation of liver and stomach were random Ly divided into group 1,group 2,group 3 and control group(n=35),which were treated with Xuanjin Jiangni Decoction,electroacupuncture at Zusanli(ST36),Xuanjin Jiangni Decoction combined with electroacupuncture at Zusanli(ST36)and omeprazole respectively.After 8 weeks,we observed the clinical efficacy,plasma gastrointestinal hormoneand endoscopy of two groups.Results:1.The total clinical effective rate of the four groups were 68.57%,57.14%,91.43%and 71.43%,respectively.The clinical effect of group 3 was better than that of the other groups(P<0.05).2.Four groups can effectively increase plasma motilin and Ghrelin.The effect of group 3 was better than that of the other groups in increasing plasma(P<0.05).3.There was no significant difference among four groups in improving esophageal mucosa inflammation(P>0.05).Conclusion:Xuanjin Jiangni Decoction combined with electroacupuncture Zusanli(ST36)may increase plasma gastrointestinal hormone and promote gastric emptying to relieve the clinical symptoms and repair the esophageal mucosal inflammation of patients with reflux esophagitis with the type of heat stagnation of liver and stomach.
