高强度超声聚焦刀联合复方苦参注射液治疗局部晚期胰腺癌的疗效

摘　　要：目的:评价复方苦参注射液联合高强度超声刀治疗局部晚期胰腺癌的临床疗效及不良反应。方法:将85例局部晚期胰腺癌患者随机分为单纯超声刀治疗组(A组)42例和复方苦参注射液联合超声刀治疗组(B组)43例。分别于治疗后1、3、6、9、12个月行增强CT扫描及CA199检查,此后每6个月复查1次,共随访24个月。观察两组疗效,并进行生存曲线分析。结果:临床受益率A组为52.38%,B组为74.42%(P=0.035);中位生存时间A组为(11.30±1.62)个月,B组为(15.40±2.62)个月;平均生存时间A组为(14.36±1.30)个月,B组为(18.86±1.69)个月。两组患者均按计划顺利完成治疗,未出现明显皮肤灼伤、胃肠穿孔坏死、梗阻性黄疸、胰漏及消化道出血等严重并发症。结论:超声刀联合复方苦参注射液治疗局部晚期胰腺癌有效、安全,不良反应可控,尤其在缓解癌痛方面具有良好效果。Objective:To evaluate the efficacyof high-intensity focused ultrasound combined with Compound Sophora Flavescentis Radix(Kushen)Injection in the treatment of locally advanced pancreatic carcinoma.Methods:A total of 85 patients with locally advanced pancreatic carcinoma were randomly divided into high-intensity focused ultrasound treatment group(group A)42 patients and high-intensity focused ultrasound combined with Compound Sophora Flavescentis Radix(Kushen)Injection treatment group(group B)43 patients.Enhanced CT scan and CA199 examination were performed at 1,3,6,9 and 12 months after treatment.The patients were followed up every 6 months for a total of 24 months.The curative effect of the two groups was observed and the survival curve was analyzed.Results:The clinical benefit rate was 52.38%in group A and 74.42%in group B(P=0.035).The median survival time was(11.30±1.62)months in group A and(15.40±2.62)months in group B.The mean survival time was(14.36±1.30)months in group A and(18.86±1.69)months in group B.All patients in the two groups completed the treatment according to the plan without serious complications such as obvious skin burns,gastrointestinal perforation necrosis,obstructive jaundice,pancreatic leakage or gastrointestinal bleeding.Conclusion:The high-intensity focused ultrasound combined with Compound Sophora Flavescentis Radix Injection in the treatment of locally advanced pancreatic carcinoma is effective and safe,especially in alleviating cancer pain.
