摘 要:目的研究轴性高度近视白内障患者行超声乳化吸除联合囊袋内人工晶状体(IOL)植入术后眼轴长度(AL)、前房深度(ACD)、角膜曲率(K)的变化,对术后屈光误差(MFE)的影响。方法将54例(54只眼)行白内障超声乳化吸除联合囊袋内折叠式IOL植人术的患者依据眼轴长度分2组:A组27例(27只眼)轴性高度近视,B组27例(27只眼)正常眼轴。测量术前1d和术后3月的AL、ACD、K、视力。SPSS19.0行统计学分析。结果A、B两组间的手术前后眼轴长度变化(△AL)和前房深度变化(AACD)的均值差异均有统计学意义(t△AL=5.536,t△ACD=5.526,P<0.01)。A、B两组的△AL与△ACD均呈正相关(rA=0.855,rB=0.639,P<0.05)。A、B两组的术前AL、△AL、△ACD与MFE均呈正相关(rA=0.874、0.877、0.858,rB=0.428、0.766、0.862,P<0.05)。A组△AL的标准化回归系数是△ACD的1.32倍,B组△ACD的标准化回归系数是△AL的1.73倍。结论10L植人术前后眼轴长度的变化是轴性高度近视患者白内障术后产生屈光误差的主要因素;前房深度的变化是正常眼轴患者白内障术后产生屈光误差的主要因素。Objective To study the changes in axial length(AL),anterior chamber depth(ACD),average comeal curvature(K)and the affect of postoperative mean absolute refractive error(MFE)of cataract patients with high myopia.Methods Fifty-four cases(54eyes)patients with phacoemulsification&intraocular lens implantation were divided into two groups according to axial length.Group A:27cases(27eyes)cataract patients with axiality high myopia;Group B:27cases(27eyes)cataract patients with normal axis length.Their preoperative and postoperative(3 months)axis length,anterior chamber depth,average comeal curvature and diopter were measured.Results The change of axis length(△AL)and anterior chamber depth(△ACD)between group A and B was statistical significant(t△AL=5.536,t△ACD=5.526,P<0.01).△AL was positively correlated with △ACD(rA=0.855,rB=0.639,P<0.05).Preoperative AL,△AL and △ACD of group A and B were positively correlated with mean absolute refractive error(rA=0.874/0.877/0.858,rB=0.428/0.766/0.862,P<0.05).The standardized coefficients of △AL was 1.32 times than that of △ACD for high myopia group,while the standardized coefficients of △ACD was 1.73 times than that of △AL for normal group.Conclusions The change of ocular axial length is the main factor affecting mean absolute refractive error after cataract surgery in patients with long-axial high myopia.While the change of anterior chamber depth is the main factor affecting mean absolute refractive error in patients with normal optic axis.
