眼内注气术治疗简单原发性视网膜脱离及预后分析

摘　　要：目的评估单纯玻璃体腔注气治疗原发性视网膜脱离(RRD)的疗效性。方法对27例眼内注气治疗的RRD患者进行回顾性分析。所有患者于表面麻醉下行眼内玻璃体腔注气术,术后依据裂孔位置保持特定头位,采用激光光凝术封闭视网膜裂孔。结果27例患者,年龄25~72岁,术前BCVA0.02~0.8,病程3~45 d,裂孔数1~3个,术后随访3年。所有患者手术顺利,24例(88.89%)首次手术后视网膜复位。初次手术视网膜未复位者,再次或者多次手术后视网膜复位率100%。首次手术复位者中,8例出现视网膜脱离复发,复发率33.33%。其中,3例为原裂孔未封闭,其余5例为新发生裂孔,分别行巩膜扣带术或玻璃体切割术。末次随访BCVA(0.12~0.8),21例(77.8%)术后视力提高;3例(11.1%)无变化;3例(11.11%)视力下降。7例黄斑脱离,其中5例(71.43%)术后视力提高。结论对于原发性视网膜脱离(RRD),单纯眼内玻璃体腔注气是一种简便易行的手术方法,适应证内可获得较好的疗效性。术后视网膜脱离复发率相对较高,但不影响再次手术。Objective To evaluate the efficacy of intravitreal vitreous injection in the treatment of primary retinal detachment(RRD).Methods A retrospective analysis was performed on 27 eyes with intraocular gas injection(RRD).All patients underwent anesthesia under the intraocular vitreous injection.After operation,the patients were asked to maintain the specific head position according to the location of the hole.The laser photocoagulation was used to close the retinal hole.Results Of 27 patients with preoperative BCVA 0.02~0.8 were followed up for 2 years,24 cases(88.89%)after the first injection achieved retinal reattachment.The left 3 cases were cured by other operation.Eight patients(rate:33.33%)had retinal detachment recurrence at 4 days to 4 months.IN recurrence patients,3 cases for the original hole did not closed,5 cases had new hole.BCVA in follow-up,21 patients(77.8%)had improved,3 cases(11.1%)unchanged,and 3 patients(11.11%)decreased.Conclusions For primary retinal detachment(RRD),intravitreal cavity injection gas is a simple and convenient method,and it can be used in the right patients.Postoperative retinal detachment recurrence rate is relatively high,but does not affect the re-operation.
