水蛭粉对脑梗死侧支循环代偿的临床效果研究

摘　　要：目的观察水蛭粉对颅内动脉重度狭窄或闭塞致脑梗死患者脑血管侧支循环代偿的临床效果,了解水蛭粉在脑梗死患者脑血管中的作用机理。方法55例颅内动脉重度狭窄或闭塞致脑梗死患者,随机分为对照组(27例)和研究组(28例)。对照组患者给予常规治疗,研究组患者在常规治疗基础上给予水蛭粉治疗。比较两组患者治疗前后总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、血浆脂蛋白磷脂酶A2(Lp-PLA2)水平及侧支循环代偿改善效果。结果治疗前,两组患者TC、LDL-C、Lp-PLA2水平比较差异无统计学意义(P>0.05);治疗后,两组患者TC、LDL-C、Lp-PLA2)水平均较本组治疗前降低,且研究组患者均低于对照组,差异具有统计学意义(P<0.05)。研究组患者侧支循环代偿改善总有效率为78.6%(22/25),显著高于对照组的51.9%(14/27),差异具有统计学意义(P<0.05)。结论水蛭粉对颅内动脉重度狭窄或闭塞致脑梗死患者脑血管侧支循环代偿的改善效果显著。Objective To observe the clinical effect of leech powder on collateral circulation compensation in patients with cerebral infarction caused by severe stenosis or occlusion of intracranial artery,and to understand the mechanism of leech powder in cerebral infarction.Methods A total of 55 patients with cerebral infarction caused by severe stenosis or occlusion of intracranial artery were randomly divided into control group(27 cases)and research group(28 cases).The control group received conventional therapy,and the research group received leech powder for treatment on the basis of conventional therapy.The total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),plasma lipoprotein phospholipase A2(Lp-PLA2)before and after treatment,and improvement effect of collateral circulation compensation was compared between the two groups.Results Before treatment,there was no statistically significant difference in levels of TC,LDL-C and Lp-PLA2)between the two groups(P>0.05).After treatment,the levels of TC,LDL-C and Lp-PLA2)in the two groups was lower than those before treatment,and the research group was lower than the control group.Their difference was statistically significant(P<0.05).The total effective rate of collateral circulation compensation was 78.6%(22/25)in the research group,which was significantly higher than 51.9%(14/27)in the control group,and the difference was statistically significant(P<0.05).Conclusion Leech powder shows significant improvement effect on cerebral collateral circulation compensation in patients with cerebral infarction caused by severe stenosis or occlusion of intracranial artery.
