摘 要:目的描述乳腺柱状细胞病变(CCL)的乳腺X线(MG)特征,提高对其的认识。方法回顾性分析41例经病理证实的CCLs,分析病变的MG钙化特征,并与组织病理学结果进行对照。结果 41例CCLs中柱状细胞变(CCC)26例,柱状细胞增生(CCH)4例,平坦型上皮不典型增生(FEA)11例,不同CCL的钙化形态差异有统计学意义(P=0.009),CCC以细小模糊钙化(69.2%,18/26)为主,而FEA以模糊细小钙化伴粗大钙化(63.6%,7/11)为主。结论 CCLs的X线钙化特征可以帮助鉴别FEA与其他CCL,为临床诊断提供决策支持。Objective To describe mammographic(MG)characteristics of breast columnar cell lesions(CCL) in order to improve the clinical understanding. Methods MG findings of 41 patients with pathologically proven CCLs were retrospectively analyzed. The MG characteristics of calcification in CCLs were compared with histopathological findings. Results 41 CCLs consisted of 26 columnar cells changes(CCC),4 columnar cell hyperplasia(CCH) and 11 flat epithelial atypical hyperplasia(FEA). Morphological characteristics of calcifications among different CCLs were statistically significant(P=0.009).Fine fuzzy calcifications were commonly seen in CCCs(69.2%,18/26),while fine fuzzy calcifications with coarse calcifications were common in FEAs(63.6%,7/11). Conclusion Calcification characteristics of CCLs might be helpful in distinguishing FEAs from other CCLs,which can provide patient management decision making support.
