局部皮瓣转移治疗眼睑肿物切除术后组织缺损的应用效果及预后分析

摘　　要：目的探究分析眼睑肿物切除术后造成的组织缺损创面,使用局部皮瓣转移修复的效果及预后情况。方法选择2013年6月至2019年7月于我科住院行眼睑肿物切除治疗的78例患者作为研究对象,入组患者均为单侧眼睑肿物,利用旋转、推进等方式修复手术创面,观察术后修复效果,并随访3-12个月,观察远期对眼睑功能的影响。结果入组78例患者邻位皮瓣转移术全部成功,切口均达到Ⅰ级愈合。术后眼睑形态良好、睑缘位置正常,瞬目等眼睑功能正常,术后随访3-12个月,患者眼睑局部瘢痕程度较轻,睑缘弧度自然,美容效果良好,未出现上睑下垂、下睑缩窄、睑外翻等并发症,患者对眼睑皮瓣缺损修复满意度达100%(78/78)。结论采用邻位皮瓣转移术治疗眼睑肿物术后皮瓣缺损效果较为理想,能够满足临床对眼睑功能和美观的双重要求,术后并发症少,患者满意度高,具有较高的临床应用价值。Objective To investigate the effect and prognosis of 78 cases of orbital flap transfer in the treatment of flap defect after orbital mass.Methods Seventy-eight patients who underwent resection of orbital mass in our hospital from June 2013 to July 2019 were enrolled.All patients were enrolled in unilateral orbital masses.Comprehensive theoretical mechanics,geometry and orthopedics theory were used to study the effect of orthotopic local flaps on the repair of postoperative flap defects in orbital masses,and were followed up for 3-12 months.Results All the 78 patients with orthotopic flap transfer were successful,and the incisions reached grade I healing.After operation,the orbital shape was good,the position of the gingival margin was normal,the eyelid function was normal,and the cleft palate was completely closed.The eyes were symmetrical.There was no blood or infection under the flap during dressing change.After 3-12 months of follow-up,the degree of local scar was mild in the eyelids,the curvature of the sacral margin was natural,and the cosmetic effect was good.There were no complications such as ptosis,constriction,valgus valgus and other complications.The patient's satisfaction with the repair of the orbital flap defect was 100%(78/78).Conclusion The use of adjacent flap transfer for the treatment of orbital mass defect after operation is ideal,which can meet the clinical requirements for eyelid function and aesthetics.Postoperative complications are low,patient satisfaction is high,and it has high clinical application value.
