经临时冠牙龈成型后的上前牙种植修复负重1年后的牙龈形态学变化

摘　　要：目的分析经临时冠牙龈成型后的上前牙种植修复负重1年后的牙龈形态学变化情况,探讨牙龈组织接受临时冠干预的可行性。方法:选取2016年1月~2018年6月我院收治的接受上前牙种植修复的患者60例,种植上部修复前应用临时冠行牙龈形态诱导,在修复完成即刻与修复完成1年后比较患者的红白美学指数、龈乳头指数和患者主观满意度情况。结果:本组患者种植修复1年后的近中龈乳头、远中龈乳头、唇侧牙龈曲线、唇侧牙龈高度、根部突度,牙龈色质、PES总分均高于种植修复后即刻,差异有统计学意义(P<0.05);本组患者种植修复1年后的牙冠状态、牙冠颜色、牙冠外形轮廓、亚冠透明度、牙冠表面质地与总分与种植修复后即刻差异无统计学意义(P>0.05);本组患者的色泽满意度评分为(8.64±1.12)分,咀嚼功能满意度评分为(8.65±1.08)分,附着高度满意度评分为(8.91±0.72)分,整体美观满意度评分为(8.99±1.36)分。结论:经临时冠牙龈成型后的上前牙种植修复具备较高的可行性,应用过程中需对适应证进行严格掌握,可获取较好的近期美学效果。Objective To analyze the changes of gingival morphology of the upper anterior teethafter temporary crown and gingivoplasty,and to explore the feasibility of temporary crownintervention on gingival tissue.Methods From January 2016 to June 2018,60 patients whoreceived implant repair of upper anterior teeth in our hospital were selected.Before upper implantrepair,temporary crown was used to induce gingival morphology.The red and white aestheticindex,gingival papilla index and subjective satisfaction of patients were compared immediatelyafter repair and one year after repair.Results One year after implantation,the gingival papilla inthe near middle gingiva,the papilla in the far middle gingiva,the gingival curve in the lip,thegingival height in the lip,the root protrusion,the gingival color and the total scores of PEs weresignificantly higher than those immediately after implantation(P<0.05);the crown state,crowncolor,crown contour,crown transparency and crown surface in this group were significantlyhigher than those in the first year after implantation(P<0.05).There was no significant differencebetween texture and total score and immediate after implant repair(P>0.05);the patients in thisgroup had satisfaction score of color(8.64±1.12),satisfaction score of mastication function(8.65±1.08),satisfaction score of attachment height(8.91±0.72),and overall satisfaction score ofaesthetics(8.99±1.36).Conclusion It is feasible to implant and repair the upper anterior teethafter the temporary crown and gingival molding.In the process of application,it is necessary tograsp the indications strictly,which can obtain better aesthetic effect in the near future.
