摘 要:目的探讨慢性阻塞性肺疾病(COPD)的炎症表型与血清miR-146a和miR-155表达的相关性。方法179例研究对象根据炎症表型分为粒细胞缺乏型、嗜酸性粒细胞型、嗜中性粒细胞型、混合粒细胞型四组,采用实时荧光定量PCR法(qRT-PCR)检测血清miR-146a、miR-155表达水平,检测受试者呼出气一氧化氮(FeNO)、血清髓过氧化物酶(MPO)、C反应蛋白(CRP)水平,并分析血清miR-146a、miR-155水平与患者炎症表型的相关性。结果与粒细胞缺乏型相比,嗜酸性粒细胞型血清FeNO、miR-146a水平显著升高(P<0.05),嗜中性粒细胞型MPO、CRP、miR-155水平显著升高(P<0.05),混合粒细胞型FeNO、MPO、CRP、miR-146a、miR-155水平均显著升高(P<0.05);与嗜中性粒细胞型相比,嗜酸性粒细胞型、混合粒细胞型FeNO、miR-146a水平显著升高(P<0.05),与嗜酸性粒细胞相比,嗜中性粒细胞型、混合粒细胞型MPO、CRP、miR-155水平显著升高(P<0.05);Pearson相关性分析表明,嗜酸性粒细胞型患者中,miR-146a水平与FeNO呈正相关(P<0.05);嗜中性粒细胞型患者中,miR-155水平与MPO、CRP水平呈正相关(P<0.05),混合粒细胞型患者中miR-146a、miR-155水平与FeNO、MPO、CRP水平均呈正相关(P<0.05),粒细胞缺乏型患者中,miR-146a、miR-155水平与FeNO、MPO、CRP水平均无相关性(P>0.05)。结论血清miR-146a、miR-155水平与COPD患者炎症表型密切相关。Objective To investigate the correlation of the inflammatory phenotype of chronic obstructive pulmonary disease(COPD)with the expressions of serum miR-146 a and miR-155.Methods According to the inflammation phenotype,179 subjects were divided into four groups:granulocyte deficiency type,eosinophil type,neutrophil type and mixed granulocyte type.Real-time fluorescence quantitative PCR(qRT-PCR)was used to detect the expression levels of serum miR-146 a and miR-155,the levels of exhaled nitric oxide(FeNO),serum myeloperoxidase(MPO),C-reactive protein(CRP)of subjects were detected,and the correlations between levels of serum miR-146 a and miR-155 and inflammatory phenotype were analyzed.Results Compared with agranulocyte deficiency type,the serum levels of FeNO and microRNA-146 a in eosinophil type increased significantly(P<0.05),MPO,CRP and microRNA-155 in neutrophil type increased significantly(P<0.05),and FeNO,MPO,CRP,microRNA-146 A and microRNA-155 increased significantly in mixed granulocyte type(P<0.05).Compared with neutrophil type,the levels of FeNO and miR-146 a increased significantly in eosinophil type and mixed granulocyte type(P<0.05).Compared with eosinophils,the levels of MPO,CRP and miR-155 in neutrophils,mixed granulocytes increased significantly(P<0.05).Pearson correlation analysis showed that the levels of microRNA-146 a were positively correlated with FeNO in eosinophils(P<0.05).In neutrophils,the levels of microRNA-155 were positively correlated with MPO.The levels of microRNA-146 A and microRNA-155 were positively correlated with the levels of FeNO,MPO and CRP in patients with mixed granulocyte type(P<0.05).In patients with agranulocyte deficiency,the levels of microRNA-146 A and microRNA-155 were not correlated with the levels of FeNO,MPO and CRP(P>0.05).Conclusion The levels of serum miR-146 a and miR-155 are closely related to inflammatory phenotypes in COPD patients.
