摘  要:目的探讨经电子支气管镜(EB)穿刺植入125I粒子联合化疗治疗中央型非小细胞肺癌(NSCLC)的临床疗效。方法选择2015年1月至2017年12月我科收治的晚期中央型NSCLC为研究对象,按照数字随机法将符合入组患者分为观察组和对照组,每组各30例,观察组给予经EB125I粒子穿刺植入联合静脉化疗治疗,对照组给予单纯静脉化疗,比较两组之间的疗效差异。结果观察组患者实体瘤缓解有效率、气道狭窄再通有效率和气促指数、Karnofsky评分较对照组相比,均显著改善(P<0.05);观察组和对照组治疗后,血清癌胚抗原(CEA)、谷胱甘肽S转移酶P1(GSTP1)、热休克蛋白90α(HSP90α)含量均低于化疗前(P<0.05),且观察组治疗后血清CEA、GSTP1、HSP90α含量低于对照组治疗后(P<0.05)。结论经EB 125I粒子穿刺植入治疗晚期中央型NSCLC有效,其联合化疗治疗较单纯化疗在实体瘤缓解、改善气道梗阻、降低肿瘤恶性程度及改善患者生活质量方面效果更佳。Objective To study the clinical effect of systemic chemotherapy and electronic bronchoscop implantation radioactive 125Ⅰseeds in treatment of advanced central non-small cell lung cancer.Methods 60 patients with advanced central type lung cancer from January 2015 to December 2017 in our department were selected,and they were randomly divided into the observation group and the control group,30 patients in each group.The observation group was treated with interstitial implantation of 125Ⅰseed via bronchoscopy and systemic chemotherapy,and the control group was treated with conventional systemic chemotherapy.The curative effect was compared between the two groups.Results The solid tumor effective rate,airway narrowing recanalization rate,dyspnea index and Karnofsky score of the observation group improved more significantly than the control group did(P<0.05).After treatment,the levels of serum tumor markers,i.e.CEA,GSTP1 and HSP90αin the observation group and the control group were all lower than that before chemotherapy(P<0.05),and the levels of serum CEA,GSTP1 and HSP9αin the observation group were lower than those in the control group after treatment(P<0.05).Conclusion Electronic bronchoscopy implantation radioactive 125Ⅰseed is an effective method for central type NSCLC,which combined with chemotherapy is better than the only chemotherapy in relieving solid tumors,improving airway obstruction,reducing malignancy and improving the quality of life of patients.
