摘　　要：目的研究RORγt mRNA、NF-κB、IL-10的水平变化与肺炎严重程度的关系及预后的相关性分析。方法选取2015年2月至2018年2月期间的180例老年肺炎患者作为观察组研究对象,以同期180例在我院进行体检的健康人群为对照组。比较两组受试者的RORγt mRNA、NF-κB、IL10的水平以及患者不同疾病程度之间的差异,分析RORγt mRNA、NF-κB、IL-10水平与患者疾病严重程度的相关性,比较单项检测和联合检测效能之间的差异。结果观察组的RORγt mRNA、NF-κB、IL-10水平显著高于对照组;中高风险组患者的RORγt mRNA、NF-κB、IL-10水平显著高于低风险组患者;患者的肺炎严重程度与RORγt mRNA、NF-κB、IL-10水平相关,联合诊断效能显著高于单独检测,通过ROC曲线分析,低风险患者的RORγt mRNA、NF-κB、IL-10临界值分别为1.20、0.99、7.02ng/L,中高风险患者的RORγt mRNA、NF-κB、IL-10临界值分别为1.34、2.31、8.14ng/L。结论随着肺炎患者炎性反应的不断提升,患者的RORγt mRNA、NF-κB、IL-10水平显著升高,通过对患者的ROC曲线分析,RORγt mRNA、NF-κB、IL-10的临界值可作为临床治疗以及诊断的参考。Objective To study the relationships between the levels of RORγt mRNA,NF-kappa B and IL-10 and the severity of pneumonia,as well as the prognosis.Methods 180 elderly patients with pneumonia from February,2015 to February,2018 were selected as the study group,and 180 healthy people who underwent physical examination in our hospital during the same period were selected as the control group.The levels of RORγt,NF-kappa B and IL-10 were compared between the two groups,and the correlation between the levels of RORγt,NF-kappa B and IL-10 and the severity of the disease was evaluated.Results The levels of RORγt,NF-kappa B and IL-10 in the observation group were significantly higher than those in the control group,also the levels of RORγt,NF-kappa B and IL-10 in the high-risk group were significantly higher than those in the low-risk group.The severity of pneumonia among patients was correlated with the levels of RORγt,NF-kappa B and IL-10,and the combined diagnostic efficacy was significantly higher than that of the single detection.The critical values of RORγt mRNA,NF-kappa B and IL-10 in low-risk patients were 1.20,0.99 ng/L and 7.02 ng/L,respectively.The critical values of RORγt mRNA,NF-kappa B and IL-10 in high-risk patients were 1.34,2.31 ng/L and 8.14 ng/L,respectively.Conclusion With the increase of inflammatory response in patients with pneumonia,the levels of RORγt mRNA,NF-kappa B and IL-10 are significantly increased.The ROC threshold values of RORγt mRNA,NF-kappa B and IL-10 can be used as a reference for clinical treatment and diagnosis of patients with pneumonia.
