摘  要:目的探讨激光协同酪蛋白磷酸肽钙磷复合体(CPP-ACP)对乳牙早期釉质龋再矿化的影响。方法收集2016年9月至2018年9月在阿坝藏族羌族自治州人民医院口腔科治疗因乳牙滞留而拔除的乳前牙50颗,人工龋造模成功后按照治疗方法不同将乳牙分为脱矿组(A组)、单纯激光组(B组)、单纯CPP-ACP组(C组)、先激光后CPP-ACP组(D组)、先CPP-ACP后激光组(E组)5组,每组10颗乳牙。采用能量色散X线光谱仪测定乳牙的磷和钙摩尔百分比含量;使用显微硬度测量仪对乳牙脱矿前后及再矿化后釉质表面硬度进行测量;使用扫描电镜获取电镜成像影像学图片。结果A组、B组、C组、D组和E组的磷和钙摩尔百分比总和分别为45.46±1.12、45.70±1.21、49.83±1.34、56.91±1.44、58.02±1.42,各组比较差异有统计学意义(P<0.05),其中C组、D组和E组高于A组(P<0.05)。脱矿后和再矿化后,各组显微硬度呈升高趋势,B组、C组、D组和E组显微硬度高于A组(P<0.05),D组、E组显微硬度均高于B组和C组,E组高于D组(P<0.05),组间、时点间、组间和时点间交互作用比较差异均有统计学意义(P<0.05)。影像学图片结果显示,除A组外,其余4组均有矿物质沉积。结论激光协同CPP-ACP能够增加乳牙磷和钙摩尔百分比,增强牙齿显微硬度,促进乳牙再矿化,其促进大于单纯使用激光或CPP-ACP。Objective To investigate the effect of laser synergistic casein phosphopeptide amorphous calcium phosphate(CPP-ACP)on the remineralization of enamel caries in early deciduous teeth.Methods From Sep.2016 to Sep.2018,50 anterior teeth removed from the deciduous teeth by the Department of Stomatology of the People′s Hospital of Aba Tibetan and Qiang Autonomous Prefecture were collected,and after establishment of artificial caries model were divided into 5 groups according to different treatment methods:a demineralization group(group A),a laser group(group B),a CPP-ACP group(group C),a laser plus CPP-ACP group(group D),anda CPP-ACP plus laser group(group E),10 deciduous teeth each group.The content of phosphorus and calcium in the deciduous teeth was determined by energy dispersive X-ray spectroscopy.The hardness of the enamel surface before and after demineralization of the deciduous teeth was measured by microhardness measuring instrument.The electron microscopic image was obtained by scanning electron microscope.Results The total molar percentages of phosphorus and calcium in group A,group B,group C,group D and group E were 45.46±1.12,45.70±1.21,49.83±1.34,56.91±1.44,58.02±1.42,there were significant differences between different groups(P<0.05),Group C,group D and group E higher than group A(P<0.05).After demineralization and remineralization,the microhardness of group B,group C,group D and group E was higher than that of group A(P<0.05),group D and group E higher than group B and group C,group E higher than group D(P<0.05).There were significant differences between groups,time points,and interactions of groups and time points(P<0.05).The imaging results showed that except for group A,the other four groups all had mineral deposits.Conclusion Laser synergized with CPP-ACP can increase the molar percentage of phosphorus and calcium in deciduous teeth,enhance the microhardness and promote the remineralization of deciduous teeth,which has better effect than the use of laser or CPP-ACP alone.
