甲状腺全切除术结合中央淋巴结清扫术对分化型甲状腺癌（肿瘤直径<10mm）的疗效及预防转移

摘　　要：目的观察甲状腺全切除术结合中央淋巴结清扫术对分化型甲状腺癌(肿瘤直径<10mm)的临床疗效。方法回顾性分析我院2012年1月~2017年3月收治的分化型甲状腺癌(肿瘤直径<10mm)患者60例,将采取甲状腺全切除术的30例患者设为对照组,将甲状腺全切除术结合中央淋巴结清扫术的30例患者设为观察组,对比两组患者的治疗效果。结果两组肿瘤直径分化和分化型甲状腺癌分类比较,差异无统计学意义(P>0.05);对照组手术时间、手术出血量均显著低于观察组,差异有统计学意义(P<0.05);两组间并发症发生率差异无统计学意义(P>0.05);术后随访2年,两组均无出现永久性喉返神经损伤、甲状旁腺功能减退,观察组的淋巴结转移率明显低于对照组,差异有统计学意义(P<0.05)。结论甲状腺全切除术结合中央淋巴结清扫术治疗分化型甲状腺癌(肿瘤直径<10mm)的疗效确切,可降低患者的淋巴结转移率,同时术后并发症并无明显增加,值得临床推广应用。Objective To observe clinical curative effects of total thyroidectomy combined with central lymph node dissection in treatment of differentiated thyroid carcinoma(tumor diameter<10mm).Methods 60 patients with differentiated thyroid carcinoma(tumor diameter<10mm)who were admitted and treated in our hospital from January 2012 to March 2017 were retrospectively analyzed.30 patients who underwent total thyroidectomy were selected as the control group while 30 patients who underwent total thyroidectomy combined with central lymph node dissection were selected as the observation group.The curative effects of patients were compared between two groups.Results There was no statistically significant difference between the two groups in tumor diameter differentiation and classification of differentiated thyroid carcinoma(P>0.05).The operation time and surgical bleeding volume of the control group were significantly lower than those in the observation group,and the differences were statistically significant(P<0.05).There was no statistically significant difference in the incidence of complications between the two groups(P>0.05).After 2 years of follow-up,there was no permanent recurrent laryngeal nerve injury or hypoparathyroidism in both groups.The metastasis rate of lymph nodes in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Total thyroidectomy combined with central lymph node dissection in treatment of differentiated thyroid carcinoma(tumor diameter<10mm)has accurate curative effects.It can reduce the lymph node metastasis rate of patients and there is no significant increase in postoperative complications.It is worthy of clinical promotion and application.
