高频超声诊断上肢常见卡压性周围神经病的临床研究

摘　　要：目的应用高频超声对上肢常见卡压性周围神经病即腕管综合征和肘管综合征患者进行检查,探讨其诊断价值。方法选取上肢卡压性周围神经病患者64例和健康志愿者32例,应用高频超声对其正中神经、尺神经进行扫查,观察其神经的回声变化并测定卡压最粗处的神经左右径及横截面积。结果与对照组相比,正中神经组腕管处正中神经左右径、横截面积均显著增大(P<0.05)。与对照组相比,尺神经组肘管处尺神经左右经及横截面积均显著增大(P<0.05)。结论高频超声可清晰显示上肢神经走行,诊断病变部位,并测量卡压处数值,具有较高诊断价值。Objective To investigate the diagnostic value of high frequency ultrasound in carpal tunnel syndrome and cubital tunnel syndrome of upper limb entrapment peripheral neuropathy.Methods Sixty-four patients with upper limb entrapment peripheral neuropathy and 32 healthy volunteers were selected.The median nerve and ulnar nerve were scanned by high frequency ultrasound.The echo changes of the nerve were observed and the diameter and cross-sectional area of the nerve at the thickest entrapment were measured.Results Compared with the control group,the left and right diameter and cross-sectional area of the median nerve at the carpal canal in the median nerve group increased significantly(P<0.05).Compared with the control group,the left and right longitude and cross-sectional area of the ulnar nerve in the cubital canal of the ulnar nerve group increased significantly(P<0.05).Conclusion High-frequency ultrasound can clearly show the course of upper limb nerve,diagnose the lesion location,and measure the entrapment value,which has high diagnostic value.
