摘  要:目的系统评价益生菌预防贮袋炎的有效性及安全性。方法计算机检索PubMed、Embase、Cochrane Library、维普中文期刊数据库、万方数字化期刊全文数据库、中国知网数据库,采用RevMan 5.3软件固定效应模型对符合纳入及排除标准的随机对照研究(randomized controlled trial,RCT)进行Meta分析。结果共纳入9项RCT,样本量为353例,其中6项报道了不良反应。结果显示,预防性使用益生菌可显著降低贮袋炎发生率(OR=0.10,95%CI:0.05~0.19,P<0.00001),其中VSL#3组(OR=0.04,95%CI:0.01~0.11,P<0.00001)疗效优于非VSL#3组(OR=0.20,95%CI:0.09~0.48,P=0.0002)。两组比较,不良反应差异无统计学意义(P>0.05)。结论益生菌可有效预防贮袋炎的发生,且不良反应少,安全性高。Objective To systematically assess the efficacy and safety of probiotics to prevent pouchitis.Methods Retrieved from PubMed,Embase,Cochrane Library,VIP,WanFang database and CNKI,randomized controlled trials(RCTs)meeting the inclusion criteria and the exclusion criteria were included and further analyzed using fixed effect model of RevMan 5.3.Results 9 RCTs were included in the Meta-analysis and there were totally 353 cases in which 6 RCTs reported adverse reactions.The result showed that preventively using probiotics could decrease the incidence of pouchitis(OR=0.10,95% CI:0.05-0.19,P<0.00001).In efficacy,the VSL#3 group(OR=0.04,95% CI:0.01-0.11,P<0.00001)was much better than the non-VSL#3 group(OR=0.20,95% CI:0.09-0.48,P=0.0002).There was no significant difference on the rate of adverse reactions between two groups(P>0.05).Conclusion Probiotics can prevent the incidence of pouchitis,with few adverse reactions and high safety.
