摘 要:目的分析牙龈卟啉单胞菌fimA基因在食管癌患者和牙周健康人群口腔中的检出率和基因型分布。方法收集74例食管癌患者和135例牙周健康者口腔牙龈标本,PCR扩增fimA基因并测序,结果与标准菌株进行比对分型,并对实验数据进行统计分析。结果口腔内fimA基因PCR扩增结果为:食管癌组阳性52例(70.27%),牙周健康组阳性56例(41.48%)。食管癌组检出率明显高于牙周健康组(P<0.01)。食管癌组fimA基因测序成功49例,各型分布为:Ⅰ型2例(4.08%),Ⅰb型7例(14.29%),Ⅱ型31例(63.27%),Ⅲ型7例(14.29%),Ⅳ型2例(4.08%),Ⅴ型未检出。牙周健康组测序成功53例,各型分布为:Ⅰ型2例(3.77%),Ⅰb型14例(26.42%),Ⅱ型27例(50.94%),Ⅲ型6例(11.32%),Ⅳ型3例(5.66%),Ⅴ型1例(1.89%)。两组人群中均以fimAⅡ型占比最高,但两组人群fimA基因各型别之间构成差别无统计学意义(P=0.59)。结论食管癌患者口腔中牙龈卟啉单胞菌fimA基因的检出率明显高于牙周健康人群,但两组人群fimA基因各型别之间构成差异无统计学意义。Objective To investigate the distribution of fimA genotypes of porphyromonas gingivalis in esophageal cancer patients and periodontal healthy population.Methods The gingiva samples from 74 esophageal cancer patients and 135 oral and periodontal healthy bodies were collected respectively,and PCR was used to amplify fimA genes and to sequence them;the results were compared with that of standard strain of fimA gene and analyzd statistically.Results The positive fimA ratio in esophageal cancer patients(70.27%)was statistically higher than that in the periodontal healthy bodies(41.48%)(P<0.01).fimA genes of 49 samples of esophageal cancer patients were sequenced successfully with the distribution categories as follows:typeⅠ2 cases(4.08%),typeⅠb 7 cases(14.29%),typeⅡ31 cases(63.27%),typeⅢ7 cases(14.29%),typeⅣ2 cases(4.08%)and no detection of typeⅤ.fimA genes of 53 samples of periodontal healthy bodies were sequenced successfully with the distribution categories as follows:typeⅠ2 cases(3.77%),typeⅠb 14 cases(26.42%),typeⅡ27 cases(50.94%),typeⅢ6 cases(11.32%),typeⅣ3 cases(5.66%)and typeⅤ1 case(1.89%).The detective ratios of fimA type II genes were the highest among other fimA genotypes in both groups,but there was no significant difference statistically in all fimA genetypes between two groups.Conclusion The detection ratio of fimA gene of Porphyromonas gingivalis in esophageal cancer patients was significantly higher than that in the periodontal healthy bodies,but there was no significant difference between two groups in all fimA genetypes.
