摘　　要：目的观察自身免疫性肝炎患者外周血PBMC中FGL2表达水平及其与外周血Th17、Treg细胞的关系。方法选择158例肝病患者,其中自身免疫性肝炎(AIH)患者108例和原发性胆汁性肝硬化(PBC)患者50例,40例同期入院健康体检者作为研究对象。以细胞免疫化学检测FGL2表达情况,以流式细胞术检测外周血中T淋巴细胞亚群含量,及PBMC中Th17、Treg细胞百分比。结果AIH组、PBC组和对照组PBMC中FGL2阳性表达率分别为79.63%、24.0%和10.00%。AIH组PBMC中FGL2阳性表达率显著高于PBC组和对照组(P<0.05),PBC组和对照组PBMC中FGL2阳性表达差异无统计学意义(P>0.05)。AIH组患者CD4^+T、CD8^+T百分比细胞较PBC组和对照组升高(P<0.05),CD3^+T细胞百分比与PBC组和对照组相比,差异无统计学意义(P>0.05)。AIH组患者Th17/CD4^+T细胞较PBC组和对照组升高(P<0.05),Treg/CD4^+T细胞较PBC组和对照组降低(P<0.05)。PBC组和对照组的Th17/CD4^+T细胞和Treg/CD4^+T细胞差异无统计学意义(P>0.05)。三组受试者外周血PBMC的FGL2阳性中Treg/CD4^+T细胞显著高于FGL2阴性(P<0.05),三组受试者外周血PBMC的FGL阳性中Th17/CD4^+T细胞百分比显著低于FGL2阴性(P<0.05)。三组受试者外周血PBMC的FGL2阳性中IL-17水平显著高于FGL2阴性(P<0.05),三组受试者外周血PBMC的FGL阳性中TGF-β水平显著低于FGL2阴性(P<0.05)。结论AIH患者外周血PBMC中FGL2阳性表达率显著增高,FGL2阳性表达的AIH患者Treg细胞百分比显著高于FGL2阴性表达的AIH患者,FGL2阳性表达的AIH患者Th17细胞百分比显著低于FGL2阴性表达的AIH患者。Objective To observe the expression of FGL2 in peripheral blood PBMC and its relationship with Th17 and Treg cells in patients with autoimmune hepatitis(AIH).Methods A total of 158 patients with liver disease were enrolled for the study,including 108 patients with AIH and 50 patients with primary biliary cirrhosis(PBC).40 health volunteers at the same period were included as the control group.The expression of FGL2 was detected by cellular immunochemistry.The content of T lymphocyte subsets in peripheral blood and the percentage of Th17 and Treg cells in PBMC were detected by flow cytometry.Results The positive expression rates of FGL2 in PBMC of the AIH group,PBC group and control group were 79.63%,24.0%and 10.00%,respectively.The positive expression rate of FGL2 in PBMC of the AIH group was significantly higher than that of the PBC group and control group(P<0.05).There was no significant difference in the expression of FGL2 between the PBC group and control group(P>0.05).The percentage of CD4^+T and CD8^+T cells in the AIH group was higher than that in the PBC group and the control group(P<0.05).The percentage of CD3^+T cells was not significantly different from that in the PBC group and the control group(P>0.05).The levels of Th17/CD4^+T cells in the AIH group were higher than those in the PBC group and the control group(P<0.05),while the Treg/CD4^+T cells were lower than those in the PBC group and the control group(P<0.05).There were no significant differences in Th17/CD4^+T cells and Treg/CD4^+T cells between the PBC group and control group(P>0.05).The FGL2 positive Treg/CD4^+T cells in the peripheral blood PBMC of the 3 groups were significantly higher than the FGL2 negative cells(P<0.05),and the percentage of Th17/CD4^+T cells in the FGL positive of the peripheral blood PBMC of 3 groups was significantly lower in FGL2 negative cells(P<0.05).The FGL2 positive IL-17 content in peripheral blood PBMC of 3 groups was significantly higher than that of FGL2 negative(P<0.05).The TGF-content of FGL positive in peri
