283例儿童异常瞬目的病因分析

摘　　要：目的分析儿童异常瞬目的病因并比较不同严重程度的异常瞬目的病因构成。方法选取我院眼科门诊2016年01月至2017年12月就诊的瞬目频繁的儿童283例,依据其发病过程、治疗经过、视力、结膜、角膜检查、干眼检查、屈光检查等方面寻找病因。并按照严重程度分级所有入组的异常瞬目,比较其病因的构成。结果 283例异常瞬目的病因中包括眼表及眼睑疾病161例(56.9%)、屈光不正及视功能问题51例(18.0%)、不良用眼习惯54例(19.1%)、神经系统疾病17例(6.0%)。根据异常瞬目严重程度分级,入组患儿中轻度159例(56.2%)、中度102例(36.0%)、重度22例(7.8%),且不同严重程度的异常瞬目病因构成存在显著差异。结论儿童异常瞬目的病因较多,常见病因为各类眼表疾病,轻中度异常瞬目主要病因为眼表疾病、屈光不正和视功能异常,而严重的异常瞬目病因更多见于抽动症和睑板腺功能障碍。Objective To analyze the cause of children’s abnormal blinking and compare the pathogeny for abnormal blinking with different severity.Methods A total of 283 children with abnormal blinking in our ophthalmology clinic from January 2016 to December 2017 were included in this study. The pathogenesis was assessed based on a review of the disease history, visual acuity and examination of the cornea and conjunctiva, dry eye and refractive error. The study participants were divided into different groups based on the severity of abnormal blinking, and the etiology were compared. Results Among the 283 cases, 161 cases(56.9%) were due to ocular surface and eyelid disease,51 cases(18.0%) were due to refractive error and visual function problem, 54 cases(19.1%) were due to bad habits with the eye, 17 cases(6.0%) were due to neurological disease. According to the severity of abnormal blinking, 159 cases(56.2%), 102 cases(36.0%) and 22 cases(7.8%) were classified into the mild, moderate and sever group, respectively. There was significant difference in the etiology among these groups with different disease severity. Conclusions Abnormal blinking of children can be caused by a variety of factors, among which ocular surface disease was the most common cause. Refractive error and visual function problem were the main cause of mild abnormal blinking, while tic disorder and meibomian gland dysfunction were common pathogeny of severe cases.
