摘  要:目的探讨超声诊断甲状腺结节的声像特征及甲状腺影像报告,借助数据系统(TI-RADS)分级分析甲状腺癌的诊断价值。方法从2018年度进入山东省医学科学院附属医院就诊的甲状腺结节患者中选择350例,依据病理报告划为两组,良性组140例,恶性组210例。依TI-RADS将其分成5级并对比。结果经评级可得,1级1例,2级54例,3级60例,4a级51例,4b级89例,4c级69例,5级26例。观察恶性组时可见微钙化、形态不规则、轮廓不清晰及血流信号较多,比良性组明显。如将4a级设为分界点并加以评级,可得特异性为73.6%,敏感性为94.4%,准确性为86.1%;如将4a级排除,规定1~3级属于良性,4b^5级属于恶性,可得特异性为92.6%,敏感性为88.9%,准确性为91.1%。结论TI-RADS对超声诊断有较大帮助,实用性强。Objective To observe thyroid nodules by ultrasound and analyze the results,and to explore the role of grading system(TI-RADS)in the diagnosis of thyroid cancer.Methods A total of 350 patients with thyroid nodules who were treated in our hospital in 2018 were divided into two groups:benign group(n=140)and malignant group(n=210).According to TI-RADS,it is divided into 5 levels and compared.Results according to the rating,there were 1 in grade 1,54 in grade 2,60 in grade 3,51 in grade 4,89 in grade 4b,69 in grade 4c and 26 in grade 5.Microcalcification,irregular morphology,unclear outline and more blood flow signals were observed in the malignant group,which was more obvious than that in the benign group.If grade 4A is set as the demarcation point and rated,the specificity,sensitivity and accuracy are 73.9%,90.8%and 86.1%,respectively.If grade 4 a was excluded,grade 1 to 3 was benign and grade 4b^5 was malignant.The specificity,sensitivity and accuracy were 92.1%,91.1%and 91.1%,respectively.Conclusion TI-RADS is helpful and practical for ultrasonic diagnosis of doctors.
