切口浸润麻醉配合静脉止痛泵用于开腹胃癌根治术的镇痛效果研究

摘　　要：目的探讨罗哌卡因切口浸润麻醉配合自控静脉止痛泵用于开腹胃癌根治术的镇痛效果。方法回顾性分析2016年4月至2019年1月该院胃肠外科收治的48例行开腹胃癌根治术患者的临床资料,分为观察组(25例)和对照组(23例)。观察组给予罗哌卡因切口浸润配合自控静脉止痛泵术后镇痛,对照组给予单纯自控静脉止痛泵术后镇痛。对照两组患者静息性及运动性疼痛评分、不良反应、术后恢复、止痛泵按压数及整体满意度。结果观察组患者静息性及运动性疼痛的疼痛视觉模拟评分在开腹胃癌术后第1、6、12、24 h均明显低于对照组,术后24 h内恶心例数及止痛泵按压数均明显少于对照组,术后24 h内下床例数及整体满意度评分均明显高于对照组,差异均有统计学意义(P<0.05)。结论罗哌卡因切口浸润麻醉配合自控静脉止痛泵对控制开腹胃癌根治术的术后疼痛安全、有效。Objective To investigate the analgesic effects of incision infiltration anesthesia combined with intravenous analgesic pump in radical gastrectomy.Methods Retrospective analysis of 48 patients undergoing D2 open radical gastrectomy for gastric cancer.25 cases in the observation group were treated with incision infiltration anesthesia combined with intravenous analgesic pump,while 23 cases in the control group were treated with simple intravenous analgesic pump.The resting and exercising pain scores,adverse reactions,postoperative recovery,number of analgesic pump pressing and overall satisfaction were compared.Results The VAS scores of resting and exercising pain in the observation group were lower than those in the control group at the 1st,6th,12th and 24th hours after the operation of gastric cancer(P<0.05).The number of nausea and analgesic pump pressing in the observation group within 24 h after operation was less than those in the control group(P<0.05).The number of patients getting out of bed and the overall satisfaction score in the observation group within 24 h after operation were higher than those in the control group(P<0.05).Conclusion Incision infiltration anesthesia combined with intravenous analgesic pump is safe and effective for controlling postoperative pain after radical gastrectomy.
