结直肠癌miR-100和miR-125b表达在西妥昔单抗联合FOLFOX治疗中的临床意义

摘　　要：目的探究结直肠癌miR-100和miR-125b表达在西妥昔单抗联合FOLFOX治疗的临床意义。方法收集2014年10月至2016年10月在石嘴山市第一人民医院就诊的结直肠癌患者98例,应用实时定量PCR方法检测miR-100和miR-125b表达水平,分为miR-100低表达65例,高表达33例;miR-125b低表达71例,高表达27例,所有患者均给予西妥昔单抗联合FOLFOX化疗,随访2年,比较miR-100和miR-125b不同表达水平结直肠癌患者的临床病理表现以及化疗效果,并应用Kaplan-Meier法绘制生存曲线比较其预后情况。结果miR-100和miR-125b不同表达水平患者的肿瘤部位、病理类型、肿瘤直径差异均无统计学意义(P均>0.05),淋巴结转移、分化程度在miR-100和miR-125b不同表达水平间差异均有统计学意义(P均<0.05)。miR-100低表达组的化疗总有效率高于miR-100高表达组(58.46%vs 33.33%,P=0.019),miR-125b低表达组的化疗总有效率低于miR-125b高表达组(29.58%vs 62.96%,P=0.037)。miR-100低表达组患者的死亡风险低于miR-100高表达组[HR=0.503,95%CI(0.259~0.975),P=0.021],miR-125b低表达组患者的死亡风险高于miR-125b高表达组[HR=2.183,95%CI(1.215~3.922),P=0.027]。miR-100低表达组患者的疾病进展风险低于miR-100高表达组[HR=0.508,95%CI(0.275~0.939),P=0.014],miR-125b低表达组患者的疾病进展风险高于miR-125b高表达组[HR=2.070,95%CI(1.234~3.475),P=0.014]。结论结直肠癌miR-100低表达和miR-125b高表达患者应用西妥昔单抗联合FOLFOX治疗可获得更好的疗效。Objective To investigate the clinical significance of miR-100 and miR-125b expression in colorectal cancer patients treated with cetuximab and FOLFOX.Methods A total of 98 patients with colorectal cancer admitted to our hospital from October 2014 to October 2016 were detected by real time quantitative PCR method,among which low expression of miR-100 was found in 65 cases,high expression of miR-100 was found in 33 cases,low expression of miR-125b was found in 71 cases and high expression of miR-125b was found in 27 cases.All patients were treated with cetuximab combined with FOLFOX chemotherapy,and were followed up for 2 years.The clinicopathological features and chemotherapeutic effect of colorectal cancer patients with different expression levels of miR-100 and miR-125b were comparatively analyzed,and Kaplan Meier method was used to draw survival curve to compare their prognosis.Results There were no statistical differences in tumor location,pathological type and tumor diameter among the patients with different miR-100 and miR-125b expression levels(P all>0.05),there were statistical differences in the degree of tumor elifferentiation and lymph node metastasis among the patients with different miR-100 and miR-125b expression lerels(P all<0.05).The total effective rate of chemotherapy in miR-100 low expression group was significantly higher than that of miR-100 high expression group(58.46%vs 33.33%,P=0.019),and the total effective rate of chemotherapy in the low expression group of miR-125b was significantly lower than that of miR-125b high expression group(29.58%vs 62.96%,P=0.037).The risk of death in the miR-100 low expression group was significantly lower than that of the miR-100 high expression group[HR=0.503,95%CI(0.259-0.975),P=0.021],and the risk of death in the low expression group of miR-125b was significantly higher than that of the high expression group of miR-125b[HR=2.183,95%CI(1.215-3.922),P=0.027].The risk of disease progression in the miR-100 low group was significantly lower than that of the miR-
