摘　　要：目的通过检测及比较两种痴呆患者与健康对照组血清蛋白水平和基因多态性,探讨阿尔茨海默病和血管性痴呆风险在血清蛋白水平和基因多态性方面的差异及相关性。方法纳入33名阿尔茨海默病(AD)患者、64名血管性痴呆(VD)患者和31名年龄匹配的健康对照者。通过ELISA检测血清中sRAGE、AGEs、Aβ1-40和Aβ1-42水平;核酸飞行质谱检测RAGE、PON1、SORL1、CLU和CR1基因多态性;利用冗余分析等方法探讨阿尔茨海默病和血管性痴呆风险在血清蛋白水平和基因多态性方面的差异及相关性。结果与健康对照相比,AD组的AGEs和Aβ1-42水平显著增加(P<0.01);VD组和AD组的RAGE基因多态性rs12034383与健康对照组相比有显著差异(P<0.01),VD组和健康对照组的CR1基因多态性rs2071288之间存在显著差异;冗余分析显示sRAGE、AGEs和Aβ水平以及RAGE、PON1、SORL1、CLU和CR1基因多态性与阿尔茨海默病和血管性痴呆有关。结论阿尔茨海默病与血管性痴呆风险在血清蛋白水平和基因多态性方面存在一定差异,同时sRAGE、AGEs和Aβ表达和RAGE、PON1、SORL1、CLU和CR1基因多态性的相关性可能是两种疾病发病的原因。Objective To investigate the differences and correlations between serum protein levels and gene polymorphisms in the risk factors of Alzheimer's disease and vascular dementia by detecting and comparing serum protein levels and gene polymorphisms between two dementia patients and healthy controls.Methods 64 patients with vascular dementia(VD),33 patients with Alzheimer's disease(AD),and 31 age-matched healthy controls were enrolled.Serum sRAGE,AGEs,Aβ1-40 and Aβ1-42 levels were measured by ELISA;RAGE,PON1,SORL1,CLU and CR1 gene polymorphisms were detected by nucleic acid mass spectrometry;the risk factors of Alzheimer's disease and vascular dementia's differences and correlations at serum protein level and gene polymorphisms were explored by redundant analysis.Results Compared with the healthy controls,the levels of AGEs and Aβ1-42 in AD group were significantly in⁃creased(P<0.01).The RAGE gene polymorphism rs12034383 in VD group and AD group was significantly different from healthy con⁃trol group(P<0.01).There was a significant difference between the CR1 gene polymorphism rs2071288 in the VD group and the healthy control group;redundant analysis showed sRAGE,AGEs and Aβlevels as well as RAGE,PON1,SORL1,CLU and CR1 gene polymorphisms were related to Alzheimer's disease and vascular dementia.Conclusion There was a difference in serum protein levels and gene polymorphisms between Alzheimer's disease and vascular dementia,and the correlation between sRAGE,AGEs and Aβexpression and RAGE,PON1,SORL1,CLU and CR1 gene polymorphisms may be the cause of the onset of two diseases.
