摘　　要：目的探讨彩色多普勒血流成像(CDFI)评估老年分化型甲状腺癌(DTC)颈部淋巴结转移的临床价值。方法选择2016年1月至2019年9月大庆油田总医院普外科收治且经手术病理证实的老年DTC患者127例,术前均行甲状腺CDFI检查,根据术后病理结果分为颈部淋巴结转移组(n=71)和非转移组(n=56),比较2组CDFI的血流参数。结果转移组的PSV、PI和RI均明显高于非转移组(P<0.05)。PSV、RI与老年DTC颈部淋巴结转移呈正相关(P<0.05)。结论PSV、PI和RI与老年DTC颈部淋巴结转移呈正相关具有显著相关性,对术前诊断淋巴结转移有较高的诊断价值。Objective To stduy the clinical value of CDFI in evaluate cervical lymph node metastasis of differentiated thyroid carcinoma(DTC)in elderly patients.Methods 127 elderly DTC patients admitted to the general surgery department of Qaqing oilfield general hospital from January 2016 to September 2019 and confirmed by surgical pathology were selected.All patients received preoperative CDFI examination in thyroid,and were divided into the cervical lymph node metastasis group(n=71)and the non-metastatic group(n=56)according to the postoperative pathological results.Blood flow parameters of CDFI in the two groups were compared.Results PSV,PI and RI of the metastatic group were significantly higher than those of the non-metastatic group(P<0.05),PSV,PI and RI were positively correlated with DTC cervical lymph node metastasis in the elderly(P<0.05).Conclusion PSV,PI and RI are positively correlated with DTC cervical lymph node metastasis in the elderly,which is of high diagnostic value for preoperative diagnosis of lymph node metastasis.
