摘 要:目的:回顾并分析甲状腺微创疗法包括化学消融(无水乙醇硬化)和热消融(射频消融和微波消融)治疗甲状腺结节的安全性及疗效。方法:收集2015年6月-2018年6月于江苏太仓市第一人民医院行微创治疗的甲状腺结节患者108例,共136个甲状腺结节。根据结节囊实性占比的差异,分别采用化学消融法及热消融法。术后1个月、3个月、6个月及12个月时记录消融区的超声表现,计算结节体积改变及缩小率,同时记录并发症发生情况,并分析两种消融方法治疗的临床疗效。结果:136个甲状腺结节均行微创治疗,消融术后消融区平均体积较消融前均明显减少,各时间体积变化与术前相比差异均有统计学意义(P<0.05)。结论:甲状腺结节的微创治疗作为甲状腺结节治疗的新技术,具有良好的临床应用前景。Objective: To investigate the safety and efficacy of chemical ablation(percutaneous ethanol injection) and thermal ablation(including radio frequency ablation and microwave ablation) in the treatment of thyroid nodules. Methods: 108 patients(total of 136 thyroid nodules) with thyroid nodules from Jun. 2014 to Jun. 2018 treated by minimally invasive treatment were analyzed. According to the difference in the solid proportion of nodular capsules, chemical ablation and thermal ablation were carried, respectively. The changes of ablation area were followed up at 1 month, 3 months, 6 months, and 12 months after surgery. The nodule volume change and reduction rate were calculated, and complications were recorded. The clinical efficacy of two ablation methods was anlyzed. Results: All the 136 nodules were successfully treated once. The average volume of ablation area was significantly decreased after ablation. The volume change of each time was statistically different(P<0.05). Conclusion: Minimally invasive treatment of thyroid nodules as a new technology for the treatment of thyroid nodules, it has good clinical application prospects.
